What You’ll Do

As a Skilled Home Health LPN, you’ll provide high-quality nursing care to patients in their homes under the direction of an RN or physician. You’ll play a key role in helping patients stay safe, comfortable, and out of the hospital.

  • Deliver skilled nursing care according to the plan of care and physician orders

  • Support patients and caregivers with education and self-care techniques

  • Observe, assess, and communicate changes in patient condition to the RN Case Manager

  • Document care accurately and timely in the EMR

  • Promote patient safety and help prevent avoidable hospitalizations

  • Build trusting relationships with patients, families, and care team members

What We’re Looking For

  • Graduate of an accredited nursing program

  • Active LPN license in the state

  • At least 1 year of clinical experience (home health or hospice experience preferred)

  • Comfortable working independently in the field

  • Valid driver’s license, reliable transportation, and ability to travel within Elkhart & LaGrange Counties

  • Ability to meet physical demands of the role
