 You will serve in a senior clinical leadership role responsible for the oversight, coordination, and clinical supervision of pediatric home nursing services for medically fragile and technologically dependent children, including ventilator-, tracheostomy-, and gastrostomy-tube-dependent patients.

Your responsibilities will include, but are not limited to:

  • Clinical supervision, mentorship, and performance oversight of nursing staff (RNs and LPNs) supporting an average caseload of approximately 30 pediatric patients

  • Oversight of patient care plans, clinical documentation, and care delivery to ensure compliance with IDPH, IDHS, Medicaid, payer requirements, and internal policies

  • Conducting clinical assessments, supervisory visits, and nursing competency validations

  • Supporting new case openings, care transitions, and coordination with intake and staffing teams to ensure timely, safe, and fully staffed service delivery

  • Participating in quality improvement initiatives, internal audits, corrective action plans, and regulatory surveys

  • Serving as a clinical escalation point for complex patient care issues and supporting clinical decision-making

  • Supporting recruitment, onboarding, orientation, and ongoing education of nursing staff

  • Ensuring patient safety, staff competency, family satisfaction, and regulatory compliance remain at the forefront of all operations

Required Qualifications • At least 2 years experience in health care, including at least 1 year of experience in care management, managed care, and/or experience in a medical or behavioral health setting, and at least 1 year of experience working with persons with disabilities, chronic conditions, substance abuse disorders, and long-term services and supports (LTSS), or equivalent combination of relevant education and experience. • Licensed Clinical Social Worker (LCSW), Licensed Master Social Worker (LMSW), Advanced Practice Social Worker (APSW), Certified Health Education Specialist (CHES), Licensed Professional Counselor (LPC), Licensed Professional Clinical Counselor (LPCC), Licensed Marriage and Family Therapist (LMFT, Doctor of Psychology (PhD or PsyD) or equivalency based on state contract, regulation, or state board licensing mandate.
