Position Summary
The Registered Nurse (RN) is responsible for overseeing non-medical home care services for clients in their homes. This role includes conducting supervisory visits, developing and updating care plans, supporting caregivers in the field, monitoring client wellness, and collaborating with families and healthcare professionals to ensure exceptional care delivery.

What You'll Do
Client Care & Clinical Oversight
Conduct supervisory visits every 30–90 days based on regulations and client needs
Perform client assessments and develop individualized care plans
Monitor client wellness and identify changes or risks that may require intervention
Observe and report changes in physical, cognitive, or emotional condition
Educate clients and families on wellness, safety, and ongoing care needs
Caregiver Support & Quality Assurance
Provide oversight and support to CNAs and PCAs in the field
Evaluate caregiver performance and adherence to care plans
Assist with problem-solving and clinical guidance for care staff
Help maintain high standards of care and client satisfaction
Documentation & Coordination
Maintain accurate and timely clinical documentation
Collaborate with office staff to coordinate scheduling and continuity of care
Communicate effectively with physicians, families, hospitals, and community partners
Participate in quality improvement initiatives and team meetings

Qualifications
Active Virginia Registered Nurse (RN) license
Experience in home care, hospice, personal care, or community health preferred
Strong assessment and communication skills
Ability to work independently and manage time effectively
Compassionate, professional, and client-focused approach
Reliable transportation and ability to travel throughout Chesterfield, Richmond, Tri-Cities, and surrounding areas
Schedule & Work Environment
Full-time position
Primarily field-based with occasional office collaboration
Flexible scheduling
Some occasional evening or weekend visits may be required based on client needs
