While an interdisciplinary care team provides medical care to patients in long-term care facilities, you will have the opportunity to impact in a positive way the life and wellbeing of the elderly population.

We are currently seeking Wound Care Nurse Practitioners to provide wound services to patients living in long-term care facilities.

WOUND NP ROLE AND RESPONSIBILITIES:

  • Provide wound care services for long-term care patients residing in skilled nursing facilities.

  • Prescribe appropriate medications, including controlled substances in accordance with OH Law.

  • Order, review, and/or follow-up with treatments and diagnostic testing.

  • Communicate with the nursing home staff on a regular and as needed to discuss changes in condition.

  • and/or the care plan.

  • Collaborate with the primary care physician.

WOUND NP QUALIFICATIONS AND EDUCATION REQUIREMENTS

  • Current, unrestricted NP license in the state of Ohio.

  • Board Certification.

  • DEA licensure (Optional)

  • Clinical background in adult, family, or geriatric specialties.

  • Clinical background of wound care; including but not limited to chronic, debilitating, stalled, or infected wounds

  • Must be comfortable with diagnosing and treating diabetic, arterial, venous, and mixed etiology wounds or Kennedy Ulcers.

  • Must be comfortable staging pressure injuries according to the NPIAP

  • Must be familiar with wound care best practice and advanced wound care products; collagens, alginates, foams, silvers, etc.

  • Must have experience with wound vacs and other DME, such as pressure relieving/ reducing devices

  • Strong preference to having wound certification or be open to getting certified 

  • Experience working in a long-term care setting.

  • Ability to work with an EMR.

  • Ability to work within an interdisciplinary care team to coordinate patient care

Job Description The Advanced Practice Provider (APP) is an Advanced Practice Registered Nurse who may also be licensed as a Certified Nurse Midwife (CNM) and has been prepared with the appropriate graduate level education and authorized to practice and prescribe as such. The APP may provide a full range of primary care health women’s health services, including but not limited to pregnancy care, menopause care, annual gynecological care, and as directed by their state designated scope of practice. APP will be responsible for interpreting fetal heart rate tracings, performing rupture of membranes and placement of internal monitors if necessary. APP may also assist during caesarian sections or be involved in vaginal deliveries. APP provides the physical, mental, and emotional support that the mother needs post-partum. Ed & Ob Triage Responsibilities APP may be responsible for evaluating gynecology consults in the ED in conjunction with a collaborating physician. Evaluate and treat pregnant patients who present in OB triage as needed, this includes but is not limited to monitoring for labor, reading and interpreting NST’s, diagnosing and managing common pregnancy conditions. Qualifications EDUCATION: Advanced Practice Registered Nurse (APRN) or Certified Nurse Midwife (CNM) must have a Master’s or Doctorate degree from an accredited program. YEARS OF EXPERIENCE: 2 – 3 years of Women’s Health or Certified Nurse Midwife experience preferred. EQUIRED SKILLS AND KNOWLEDGE: The APP demonstrates the knowledge, abilities, and skills to provide culturally specific patient care and education.
