Job Responsibilities:

-Manage all aspects of patient care from initial admission assessment to weekly visits, to discharge when goals are met.

-Coordinate patient care with their physician as well as other disciplines involved in care (PT,OT,ST,HHA)

-Upon admission, develop plan of care tailored to patient’s individual needs, including establishing goals and discharge planning.

-Revise the plan of care as necessary, depending on patient status and any changes.

-Communicate with physician regularly on patient condition and to request any orders necessary.

-Provide direct nursing care and education based on the care plan.

-Completes all assessments and nursing notes timely and thoroughly.

-Collaborate with other home health disciplines for the best patient outcomes, which includes attending case conference meetings regarding patient overall care.

-Supervision of HHA and LPN visits, when applicable.

-Communicate effectively with other team members, including administrative supervisors regarding scheduling, patient updates, or any other daily updates.

-Participate in on-call rotation when applicable.

 

Qualifications:

Graduate of an accredited school of nursing with Associate/Bachelor’s degree.

Current unencumbered RN license and CPR certification

1 year of home health experience required

1-2 years of acute care RN experience required.
