Hospice Nurse Practitioner

We are seeking a dedicated Hospice Nurse Practitioner to perform Face-to-Face (F2F) encounters essential for hospice eligibility certification and recertification. In this role, you will gather comprehensive clinical information, perform detailed assessments, and collaborate closely with hospice physicians and interdisciplinary teams to ensure quality patient care and regulatory compliance. Key Responsibilities: Perform and complete physical assessments for hospice F2F encounters within CMS-established regulatory timeframes. Document clinical findings accurately and attest F2F encounters in patient medical records. Utilize Local Coverage Determination (LCD) criteria per hospice policy and regulatory requirements. Communicate clinical findings, review treatment plans, lab results, diagnostics, and medications with certifying physicians. Collaborate effectively with physicians, interdisciplinary group (IDG) members, patients, caregivers, and families. Provide clinical recommendations to the IDG based on F2F assessment findings. Conduct assessments in the patient’s place of residence, including home, skilled nursing, or other settings. Notify the hospice agency promptly if unable to complete F2F visits as scheduled. May provide F2F coverage for multiple provider numbers with approval. Provides preoperative and postoperative care including patient education, by maintaining communication with patients and their families. Assists the surgeon during operations by holding retractors, operating laparoscopic cameras, assist in robotic procedures, suturing wounds, using surgical staplers, providing suction or anything else that the surgeon may need. Assists surgeon by maximizing patient volume, monitoring and contacting high risk patients, returning patient calls in a timely manner, and providing follow up. Coordinates with oncology and radiation oncology for our cancer patients. Interprets radiology results to determine surgical plan. Develops with the patient and/or caregivers, a mutually acceptable cost-aware plan of care that maximizes health potential. Formulates a treatment plan which includes ordering diagnostic tests, selecting appropriate pharmacologic and non-pharmacologic interventions, develops a patient education plan, and obtains appropriate consultation or referral. Consults with physician appropriately regarding patient care issues and concerns. Communicates clinical findings to, and works collaboratively with, physicians to develop patient treatment and education plans. Conducts a small lesion clinic by performing minor procedures and see post operative patients independently in the office. Reviews all referrals for soft tissue tumors, skin lesions and properly delegates to self or surgeon for excision. Completes all discharge summaries for two physicians. Demonstrates productivity and initiative by organizing patient care, delegating to appropriate staff, prioritizing work load and seeking opportunities to improve clinical processes and staff/patient education. Hosts floor specific educational seminars for nursing education in the hospital setting. Helps implement new protocols and delegates tasks to floor nursing managers in relation to surgical care. What are the requirements for the job? Master’s Degree in Nursing and maintaining a board/association certification Licensed by the State of Missouri as a Registered Nurse to practice as an Advanced Registered Nurse Practitioner.
