Position Summary
The Admissions coordinator LPN LVN serves as an access point for hospice services by coordinating prescribed services with the physician, payor, patient/caregiver, and provider to ensure that the services are provided in a timely and coordinated fashion. Additionally, the Admissions coordinator LPN LVN reviews patient referrals for appropriateness and compliance with Hospice Conditions of Participation, federal, state and local rules and regulations.
Position Specific Responsibilities
- • Respond to incoming calls from patients, families, physicians, facility staff, and others in the community. Provide general information about home health/hospice services in response to inquiries; direct callers to appropriate resources if seeking services that home health/hospice does not provide.
• Take referrals, collecting sufficient information to determine a prospective patient’s appropriateness for hospice, DME requirements and other services to be coordinated at the time of admission.
• Arrange the appointments for hospice admission with patients, families, and caregivers while coordinating DME, infusion services, and interpreters as needed.
• Collaborate with ministry staff for assignment of appropriate personnel for newly admitting patients.
• Document communications in EMR related to referral and admission coordination accurately and timely according to established standards; ensure that the Access Services Department’s data collection needs are met and assist in tracking/collating referral and admission information and statistics.
• Work with hospital and extended care facility managers and discharge planners, physician offices, and other referral sources to plan and organize hospice services for patient discharges.
• Work closely with insurance verification and authorization employees as needed to investigate and document insurance eligibility and benefits and required authorizations for all referrals. Communicate information to patients/caregivers, physicians, and providers and assists in making alternative arrangements as dictated by insurance coverage and benefits.
• Collaborate and seek input from Clinical Managers when patient’s psychosocial and/or spiritual issues indicate that assessment and interventions may be needed quickly following admission.
• Consults with Access Services RNs regarding patient eligibility and complex issues with support from department manager as appropriate.
• Maintain positive working relationships with referral sources, outside agencies and co-workers; contribute to the identification strategies to improve agency-responsiveness to referral sources.
• Contribute to overall home health/hospice team function through effective time management and communication; utilize critical clinical thinking and problem-solving skills to address team issues.
• Responsible for maintaining awareness of professional Scope of Practice.
• Adhere to PSCS and other Providence policies and procedures, and applicable regulations.
• Participate in Continuous Quality Improvement activities, identify related issues and implement recommended changes as appropriate.
Education and/or Experience
- Required - Degree from an accredited Licensed Practical/Vocational Nurse program.
- Required - 2 years Nursing experience.
- Preferred - Prior Hospice, Homecare, Long Term Care, medical/surgical, and/or oncology experience.
Certifications, Licenses, and Registrations
- Required upon hire: California LVN (Vendor Managed)
- Required upon request: Driving may be necessary as part of this role. Caregivers are required to comply with all state laws and requirements for driving. Caregivers will be expected to provide proof of driver license and auto insurance upon request. See policy for additional information.
Physical Demands and Work Environment: The demands of this role necessitate a team member to effectively perform essential functions. Adaptations can be made to accommodate team members with disabilities. Regular standing, walking, and manual dexterity are fundamental, along with the ability to lift and move objects up to 50 pounds. Visual acuity requirements include close and distance vision, color and peripheral vision, depth perception, and the ability to adjust focus. In a healthcare setting, exposure to bodily fluids, infectious diseases, and conditions typical to the field is expected. Routine use of standard medical equipment and tools associated with clinical care is essential. Compensation range: $34.70 - $53.86
The amounts listed are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities
