POSITION SUMMARY The purpose of your job position is to provide each resident with routine daily nursing care in accordance with the resident’s assessment plan along with current federal, state, and local standards that govern the facility, and as directed by your supervisors. ESSENTIAL DUTIES AND RESPONSIBILITIES Ensuring that all nursing care is provided in privacy Assisting therapy department with resident mobility Reporting any changes in residents condition immediately to the supervisor Helping the van driver with the transporting of residents to and from appointments Proper lifting and transitioning residents from wheelchair to bed, bed to chair, etc. Helping residents, sit, stand and walk Timely reporting of change in resident’s condition to the Nurse Supervisor Protecting each residents file and data with confidentially per HIPAA standards Abiding with all facility policies and procedures including not disclosing user ID codes and passwords Following Infection and Control policies Reporting any occupational exposures to blood, body fluids, or other hazardous materials to a supervisor immediately Attend annual facility in-service training programs Other duties as assigned, including CNA back-up coverage (answering call lights, making residents comfortable, assisting in feeding residents, helping residents with grooming, etc.) Every effort has been made to identify the essential functions of this position. Position Overview: The CNA is a member of the community nursing team whose responsibility is to assist professional nursing personnel by delivering direct hands-on nursing care to ambulatory and non-ambulatory residents on a daily basis. The CNA is also responsible for assisting the residents to achieve the highest level of functioning while living as normal a lifestyle as possible. Key Responsibilities: • To perform or assist the resident with completing Activities of Daily Living (ADL). • Respond to residents call lights to provide maximum comfort, safety and privacy. • Transport residents by ambulation or by wheelchair to areas of community (dining room, lobby and activities). • Report any significant findings/observations to nurse immediately. • Assist residents on admission, transfer and discharge to make a smooth transition. • Responsible for the safekeeping of residents’ personal possessions. • Keep resident rooms neat and safe. • Follow proper infection control techniques • Respect the rights of each resident, particularly privacy and confidentiality. • Re-teach resident to perform ADL skills. • Provide restorative care to prevent pressure sores, contractures, and complications. • Participate in Continuous Quality Improvement and ensures follow-up occurs. • Converse with resident and family; assist them on adjustment to the community. • Ensure adequate hydration and nutrition of resident; report deviations from norm to the Charge Nurse promptly. • Perform blood pressure, pulse, respirations, intake/output and weights accurately. Qualifications: • Certified and state tested as a Certified Nursing Assistant in the state employed; certificate must be active, valid, and in good standing.
