The caregiver must follow the client’s care plan and use any required gait belt, Hoyer lift, sit-to-stand lift, or other approved transfer device only after receiving proper training and authorization. If a transfer requires two caregivers or cannot be completed safely, the caregiver must not attempt it alone. The caregiver must keep the client safe and contact the Helping Hearts office immediately for assistance and further direction. Applicants must be able to perform the essential transfer responsibilities of this position safely and consistently, with or without reasonable accommodation. Proactive Fall Prevention and Safety Monitoring Because this client is at significant risk for falls, the caregiver must remain alert, attentive, engaged, and safety-focused throughout the entire shift. This is not a passive companion assignment or a position where the caregiver can sit nearby without actively observing and assisting the client. The caregiver will be expected to: Remain aware of the client’s location, mobility, and immediate needs Provide close supervision during standing, walking, toileting, and transfers Stay within a safe assisting distance whenever the client is moving Encourage the client to request assistance before attempting to stand or transfer Maintain a clear and hazard-free environment Check for spills, cords, loose rugs, clutter, poor lighting, and unsafe furniture placement Make sure mobility devices are properly positioned and within reach Make sure the client has appropriate footwear during mobility Keep commonly used items safely within the client’s reach Remain especially attentive when the client is tired, weak, distracted, or attempting to toilet Never leave the client unattended during a task when supervision is required Avoid personal-phone use or other distractions that interfere with active supervision Immediately report changes in mobility, balance, strength, or transfer ability Immediately report every fall, near fall, injury, or safety concern Complete all required incident documentation accurately and promptly The caregiver must never attempt an unsafe lift or transfer. Concerns must be reported immediately. Caregivers must not wait until the end of the shift to report an urgent change, fall, near fall, injury, or safety concern. Small Dog in the Home The client has a small dog. Applicants must: Be comfortable entering and working in a home with a small dog Remain aware of the dog’s location when the client is walking or transferring Help prevent the dog, toys, food bowls, or leashes from becoming fall hazards Follow the client’s instructions regarding appropriate interaction with the dog Never allow the pet to interfere with the client’s care, supervision, or safety Routine pet care is not automatically part of the assignment unless it is specifically included in the client’s approved care plan. SYNERGY HomeCare

Home Care CNA – Where Being Dependable Pays Off

$18.50 - $23.50 / hour
Dear Valued Job Seeker, We’re going to skip the usual “caregiver job-posting language,” because YOU already know what caregiving is. Instead, I want to start by personally introducing myself: my name is Carly, and I am the owner of SYNERGY HomeCare of St. Charles. You already know that caregiving can mean a lot of things. It can mean helping someone with taking a shower, making them their meals, doing their laundry and some light housekeeping, giving them an occasional ride to appointments, and SO much more. You also know those small things can make ALL the difference in the world to someone. YOU are the important person who makes it possible for someone to stay in their own home. With so many home care companies out there, how do you choose the right one? Let us help you narrow it down. Ask yourself: What matters to YOU when it comes to the company you work for? Do you want… A company that understands you may have financial goals, and gives dependable caregivers the opportunity to work toward them? To be surrounded by caregivers who take their work seriously, instead of constantly picking up the pieces for people who don't? An office that actually knows who you are, understands your strengths, and pays attention to which clients and schedules fit you best? Your hard work, reliability, and performance to actually matter? THAT is the kind of culture we're building with our staff. But culture goes both ways. Are YOU willing to work hard, communicate, show up, and take your commitments seriously to be part of something more? Our Company Culture : We are NOT looking to collect as many employees as possible. We're looking for dependable caregivers we can actually build around. That means people who communicate, show up when they say they will, take ownership of their shifts, and understand that when a family hires home care, they are trusting us with THE LIVES of their loved ones. Because caregiving is such an important responsibility, I believe caregivers should be treated as members of our SYNERGY family. We're a locally owned home care company serving St. Charles, Lincoln and Warren Counties (we don't cross the river into STL). We are very intentional about who we hire and how we operate. Our Scheduling Philosophy : We understand that schedules are not ‘one-size-fits-all’. Some caregivers want full schedules, and some want a few shifts a week. Some want evenings, weekends, mornings, longer shifts, or hours around another job or school. We have a wide variety of shift options, and we match caregivers based on availability, experience, personality, and what the client actually needs. Flexibility, however, is not the same thing as lack of accountability. Our clients are often older adults, people recovering from a hospital stay, people living with dementia, and families in high-acuity hospice situations. A call-off in home care is NOT like a call-off at a facility. It can mean a vulnerable person is suddenly left without any care. About this position ➤ You would serve clients in the Troy and Lincoln County area ☀️ Daytime shifts (from short visits to 12-hour shifts), full-time or part-time ✚ Looking for experienced caregivers, CNAs, HHAs What daytime care means with us: Daytime care with us includes all the usual caregiving tasks you already know, but we take it to a whole different level with more documentation and more communication than you will find at many other companies. In summary: we want YOU if you're an experienced caregiver or CNA looking for flexible work with a company that works very hard to treat good caregivers with the respect and dignity they deserve. If you want flexibility AND take your commitments seriously, dependable caregivers that work for us do not hurt for hours. We especially value caregivers who are observant, organized and comfortable in their capabilities. Apply here if my company sounds like the right fit for you because I would genuinely LOVE to hear from you! Sincerely, Carly Owner, SYNERGY HomeCare of St. Charles
