Description The Clinical Nurse is a Registered Professional Nurse who provides safe, competent quality care based on nursing theory and research to a designated group of patients and significant others. Responsibilities Patient Care 1. Overview JOB SUMMARY Plans and provides professional nursing care to assigned oncology patients. Applies oncology nursing principles through collaborative relationships with patients, and families. Responsibilities Plans, implements, and evaluates all professional nursing care given to oncology patients in the out-patient setting. Carries out direct and indirect patient care activities, including physical care, psychological support and patient teaching. On physician's orders or under physician's auspices, administers all types of parenteral chemotherapy to oncology patients in the out-patient setting; prepares parenteral chemotherapy under laminar flow hood as needed to augment pharmacy coverage. Determines and coordinates services needed by oncology patient and family (e.g., social services, financial planning); makes appropriate referral to other hospital departments and or community agencies as needed. Acts as a resource person for all oncology patients and other hospital nursing staff in area concerning, but not exclusive to oncology nursing. Suggests and implements new and improved progressive methods and techniques of oncology nursing care. Applies new developments in oncology care to the hospital situation. * Patient care responsibilities are delivered with a knowledge of patient growth and development and are appropriate to the ages of the patients served. * Maintains established departmental policies and procedures, objectives, quality assurance program, safety, environmental, and infection control standards. Description Accountable for the coordination and implementation of the Nursing Department payroll and other administrative systems, for ensuring appropriate office procedures and resources are utilized as well as provision of staff training. Responsibilities Develops procedures and administrative systems to ensure the accuracy and integrity of nursing services data such as systems documentation, management of records and processing of records. Description The Clinical Program Manager for ALIGN-CARE Geriatric Surgery Co-Management Program is responsible for overseeing the continuity of patient care. Patients will be 65 years or older, frail, community-based, considering surgery, assessing surgical risks and provide care plans to optimize health prior to surgery. Patients enrolled in the program may come from any part of Mount Sinai Health System; however, hospitals of care receipt will be both Mount Sinai Hospital and Mount Sinai Morningside Hospital. Responsibilities Oversees clinical operations within the program, using data and fact-based problem-solving techniques to improve processes and outcomes. Ensures that all clinical initiatives run according to institutional and departmental policies and in accordance with any government or regulatory requirements as applicable. Plans, develops, and/or maintains departmental standard operating procedures. Regularly reviews policies and procedures and makes changes or recommends changes to superiors as necessary. Prepare the sterile field and assists in preparing the operative site and draping the patient. Transport patients to and from operating room and performs preoperative shave preps. Assist in the preparation and aftercare of equipment and supplies. Clean unsanitary instruments, checks for defective items, and assembles instruments correctly in a container or tray, and wraps and labels instruments appropriately. Test the operation and efficiency of equipment and reports problems to the Registered Nurse. Collect and transport required operative set-up to surgical room. Assist in cleaning and restocking rooms according to established standards. Assist in taking inventory of equipment and supplies for specialty areas. Required Qualifications Successful completion of a nationally accredited surgical technologist education program and current surgical technologist certification by a nationally accredited surgical technologist credentialing organization. Participates in the application of the nursing process to an assigned group of patients. 2. Demonstrates those behaviors which reflect the scientific knowledge, judgment, and technical skills of a Licensed Practical Nurse 3. Performs job duties under supervision of a Registered Professional Nurse. 4. Demonstrates a competent level of approved skilled nursing care tasks and procedures using approved technical skills related to vital signs, pulse oximetry, venipuncture, starting standard peripheral IVs only (not midlines or long peripheral lines), urinary catheter insertion/catheter care; and simple dressings, remove/discontinue a peripheral line. 5. Administer IV fluids, provided completion of annual competency IV therapy training. 6. Monitors patients receiving blood and blood products and TPN under the direction of the RN, who administers the transfusion. 7. Makes recommendations in the application of the nursing process. 8. Participates in planning, and administers direct patient care, including activities of daily living (ADL). 9. Performs point of care testing including blood glucose. 10. Performs nursing procedures as defined in the nursing procedure manual. 11. Administers medication as defined in nursing policy. ALL routes except cannot administer medications IV push/bolus (except for a Normal Saline or heparin flush) or via a central line. LPNs do not administer chemotherapy or immunotherapy. 12. LPNs cannot administer first dose of an IV drug. The RN will administer the first dose of an IV drug and assess for side effects, after which the LPN can administer subsequent doses. 13. LPNs working exclusively in chronic outpatient hemodialysis settings may, under the direct supervision of a Registered Professional Nurse (RN) and after completing an approved hemodialysis training and competency program, perform dialysis-related tasks as permitted by the New York State Education Department (NYSED) Office of the Professions and 10 NYCRR §700.4(b). See Section D for more details regarding role of LPNs in the Outpatient Hemodialysis setting. This exception does not permit independent practice or unsupervised access to central venous lines. 14. Assists in patient assessment after the RN has completed the initial patient assessment, including pain management. 15. Carries out medical and nursing orders. Cannot accept verbal orders or carry out RN standing orders. 16. Makes, reports and records observations on assigned patients within the EMR. 17. Observes and reports any signs and symptoms of change in a patient’s condition to the appropriate person. 18. Participates in health teaching to patients and families following the RNs development of the teaching plan. 19. Establishes an environment that promotes optimum physical and emotional comfort. 20. Performs purposeful hourly care checks and supports the patient experience in accordance with Relationship Centered Care Professional Practice Model 21. LPNs in the Outpatient Hemodialysis Setting An LPN can practice in the chronic hemodialysis outpatient department under the direct supervision of an RN. The LPN will participate in educational programs on a regular basis, including initial orientation and continuing in service training to care for patients with central venous access devices (CVAD). The training should include a written examination and competency demonstration. A Hemodialysis LPN may: • Administer bolus of medications by IV push in a chronic outpatient hemodialysis treatment center, excluding first dose administration which is administered by an RN. The RN will access and care for patient’s initial catheter use. Subsequently, and following the MSHS CVAD care policy, the LPN can: • Identify and set up supplies necessary for accessing a CVAD. • Initiate treatment with a CVAD. • Draw blood specimens from a CVAD. • Perform CVAD dressing change and exit site care. • Monitor and adjust flow rates of central venous catheter access device/line. • Intervene if there is a catheter/blood line disconnection or other complications associated with a CVAD. • Provide CVAD care at the end of dialysis treatment. Ensure open and effective channels of communication with patients, family/caregivers and all health care team members involved in the pre/intra/post-care of adult cardiac catheterization patients. Procedures include (but are not limited to): angioplasty, coronary stents, balloon valvuloplasty, Swan Ganz/right heart catheterization, and pericardiocentesis. JOB DUTIES: • Transport patient to/from pre-post recovery and/or inpatient unit. • Administer intravenous and oral medications to patients. • Manage patients under procedural sedation. • Operate physiologic recorder and printer to monitor electrocardiogram and hemodynamic data. Troubleshoot equipment issues and/or initiate repair. • Operate intra-aortic balloon. Troubleshoot equipment issues and/or initiate repair. • Operate Impella. Overview The Nurse Manager is responsible for 24/7 oversight and clinical supervision of nursing staff. The manager is clinically and administratively responsible for the day-to-day oversight of all activities within the service in collaboration with the Director of Nursing/Psychiatry, to ensure delivery of quality patient care, consistent with regulatory agency standards, organizational policies, and evidence-based approaches.
