o Supervises nursing care in area of assignment. Central Park Rehabilitation and Nursing Center Registered Nurse (RN) Unit Manager whose primary duties are to plan, organize, develop and supervise the day-to-day nursing activities of a 40 bed unit. The Nurse Manager plays a key role in creating a positive patient experience. In this role, you will assume responsibility for the overall assessment, planning, implementation and evaluation of patient care on the unit to facilitate and deliver comprehensive quality nursing care and to manage the environment in which care is given. Overview To support the Mission of Helio Health, Inc., the CTI Licensed Practical Nurse will provide comprehensive care management and coordination of services to Critical Time Intervention (CTI) program enrollees. The CTI model is a time-limited form of case management that focuses on helping people with severe mental illness (SMI) and a history of homelessness during a “critical time” transitional period in their lives. CTI Licensed Practical Nurse: CTI is designed specifically to enhance the continuity of care during transition from hospital to community care.
