Job Summary:

Knowledge, Skills and Abilities:

  • Required: Graduate of an accredited school of nursing.

  • Required: Current licensure in the state of employment.

  • Demonstrates knowledge of nursing process, particularly as it relates to geriatric residents, long-term care, and rehabilitation.

Demonstrates knowledge of nursing principles for the resident in the relevant clinical setting. The Licensed Practical Nurse conducts patient assessments and implements the individualized plan of care established by the Registered Nurse to ensure that each patient’s clinical needs are met. This role delivers one‑on‑one patient care and communicates any changes in patient status to the RN Case Manager in a timely and accurate manner. The home health environment is technology‑driven; therefore, the Licensed Practical Nurse uses email as a primary communication tool and completes all documentation electronically using a laptop‑based medical record system. The patient population represents a broad range of ages, diagnoses, and levels of acuity. The Licensed Practical Nurse practices under the direction of a Clinical Team Manager.
