Department HOME HEALTH - 207151 Worker Sub Type Regular Work Shift Pay Grade Job Description The Home Health Licensed Practical Nurse provides direct patient care in the home setting as a key member of an interdisciplinary team that includes Registered Nurses, Physical Therapists, Occupational Therapists, Speech‑Language Pathologists, Social Workers, and Home Health Technicians.
