PPG Primary Care Clinic Manager

Summary

Responsible for overall practice leadership, quality outcomes, care delivery, management and financial performance of the designated primary care practices(s). Works with senior leadership, physicians, co-workers and patients in a collaborative manner, assuring all policies, procedures and programs are compatible with goals and objectives of line of business

Education

Associate’s or Bachelor’s degree in Health Science, Healthcare Administration or Business Management preferred.

Licensure/Certification

Clinical or nursing licensure a plus.

Experience

3-5 years in a leadership role in a healthcare environment, physician office preferred. Must be able to demonstrate experience related to leadership, finance, clinical operations, service excellence, and quality outcomes measurement.

Other Qualifications

Solid verbal and written communication skills required. Must be able to understand and evaluate both clinical and non-clinical roles of co-workers. Candidate will be expected to utilize LEAN methodologies for process improvement in each office while leading change and fostering care delivery under population health.

Records observations, treatments, and other pertinent information. Communicate with Medical Director, Attending Physician, branch leadership and IDG regarding findings from the visits and any measures to alleviate symptoms. Provides physical and emotional care to support the patient and family in period of crisis. Ability to triage consults by patient acuity. Performs bedside palliative care with patient/family education Ensures that all activities performed align with the vision of Elara Caring’s board of directors, executive team, and the leadership of the Hospice team. Prevents unwanted hospitalizations by meeting patient and family needs in the home. Prepares and maintains accurate patient records, charts, and documents to support sound medical practice and reimbursement for services provided. Compliance with applicable laws and regulations with respect to Collaborative Agreements. Completes accurate billing and coding activities for all patient encounters according to CMS guidelines and initiates reimbursement for medically necessary visits in accordance with applicable law and regulation. Provides training and continuing education for staff. Assists in development of clinical practice guidelines/standards in support of quality palliative care. Adheres to the practice of confidentiality regarding patients, families, staff and the organization. Collaborates/Communicates with Medical Director, attending physician, branch leadership and IDG. Performs other duties/projects as assigned. What is Required? Master’s degree in nursing and certification through a national Nurse Practitioner Licensing board. 1+ years of clinical nursing experience 1+ year post-graduate clinical nursing experience in the hospice and palliative care field preferred Prior experience in Family Medicine, Geriatrics and psychiatry preferred. Current licensure in the state of practice as a Nurse Practitioner and maintain such licensure in accordance with applicable laws and regulations. Maintain active DEA certification if state allows prescription of controlled substances by APPs 50% travel required Sit, stand, bend, lift and move intermittently and be able to lift 50-100 lbs.
