How you make a difference

The Nurse Practitioner Telehealth provides remote medical care to inmate patients through a telemedicine platform in collaboration with physician staff and onsite healthcare teams. This role conducts history and physical visits and sick call clinics using telehealth technology with support from onsite presenters. The position coordinates patient care through assessment, planning, implementation, and evaluation while following established policies and all applicable laws and regulations. Care is delivered in a manner that supports efficient, high-quality, and compliant telehealth services.

Key Responsibilities

Conduct telehealth evaluations including history and physicals and sick call visits in collaboration with physicians and onsite presenters.
Provide clinically appropriate assessments, treatment plans, and patient and staff education aligned with best practices and protocols.
Collaborate with onsite healthcare teams to prepare, conduct, and support efficient telehealth clinics.
Document all clinical findings accurately and timely in the electronic medical record while maintaining confidentiality.
Deliver remote care, participate in quality and peer review activities, and provide cross-coverage and clinical support as needed.

Qualifications & Requirements

Education

Completion of a master’s program in nursing (NPs)

Experience

Minimum of one (1) year correctional healthcare experience preferred

Licenses/Certifications

Unrestricted current license, certification, or registration in EACH state of practice - must maintain all certifications, educational requirements, licensing, etc.

Position Hours: 8:00am - 6:00pm (10 Hour Shifts) No Weekends or Holidays Department Summary: VPEC has two locations: The Vanderbilt Clinic (TVC) and One Hundred Oaks (OHO), both are fully staffed clinics with 11 Nurse practitioners, 5 registered nurses and multiple MAs and PSRs. The staff at either location performs: vitals, images, sleep apnea screening and a complete review of systems and history and physical on patients undergoing elective surgeries. The nurse practitioners performs these visits in person, while the registered nurses use a virtual phone call model. We can also arrange for lab work and x-rays to be done at the time of the VPEC visit or through a local primary care doctor for our phone call patients. We gather any outside testing and can order electrocardiograms, cardiac consults, stress tests and echocardiograms if felt needed by the attending anesthesiologist. Patients are usually referred to the clinic by one of our surgeons for medical optimization prior to the day of surgery. This process allows our team of Nurse Practitioners to ensure that preoperative patients are properly evaluated, educated and that all necessary testing and patient information are in hand before procedures. This is a free evaluation to assess the patients overall risk for surgery. This better ensures patient safety and helps prevent surgical delays and cancellations on the day of surgery. Once the patient evaluation is completed at VPEC, it becomes part of the patients electronic medical record and is accessible by any Vanderbilt medical provider. VPEC has received several significant industry accolades, including the 2014 Top Performer Award and the coveted 5-Star Award for Overall Quality of Care. This means a score in the 90th 99th percentile based on results and as compared to similar centers nationwide. The award was given to both VPEC clinic sites. The MPE will collaborate with Monogram Health’s Multi[1]Specialty Platform to leverage employed specialists to deliver in-home specialty care. Monogram Health deploys a proven risk[1]based model to ensure health equity and health equality leveraging proprietary next generation AI algorithms to predict the appropriate level of care. Aligning with the quadruple AIM, the MPE will focus to improve patient experience, population healthoutcomes, provider satisfaction and lower costs. The primary goal of each MPE to deliver exceptional outcomes through disease detection and evidence based clinical pathways and disease treatment. Reporting to the Region President, the Market Physician Executive (MPE) is a key clinical leader within Monogram Health who contributes to the development and oversight of clinical strategies, policies, programs, processes, protocols, guidelines, and operations that drive improved patient health outcomes within the market. The MPE oversees the daily clinical and business operations through delivery of direct patient care, care management services, social worker support and pharmacy services within the market. Roles and Responsibilities Clinical Guidelines Execution – 25% Know, understand, and deliver on Monogram Health’s proprietary evidenced based clinical pathways. Ensure adherence to established clinical guidelines and Monogram Model of Care. Review and approve APP, RN, SW and PharmD plans of care. Appropriate and timely patient document within Salesforce and Athena clinical activities, interventions, and tasks. Review, approve and co-sign APP encounters. Clinical and Operations Performance and Quality Improvement – 25% Overall accountability for reducing total cost of care and Medical Loss Ratio. Responsible for clinical outcomes to include, but not limited to, clinical interventions closure, inpatient/outpatient utilization, pharmacological prescribing and therapy management, multi-specialty platform and HEDIS/Gap Closure Actively lead daily high risk and concurrent review rounds. Population Health Management (PHM) – 20% Provide clinical guidance and direction to Market teams to drive Population Health Management activities, including identifying and intervening on High-Risk Patients, formulating strategies to reduce admissions/readmissions, complete quality post hospital discharge visits, and improving the quality of visits.
