Nurse Case Manager (PRC) – Fort Peck Service Unit

Location: Chief Redstone Health Center | Wolf Point, MTSchedule: Monday – Friday | 8:00 a.m. – 4:30 p.m. | Federal Holidays Off

Spectrum Healthcare Resources has an excellent opportunity for a Nurse Case Manager (Purchased/Referred Care – PRC) to support the Indian Health Service (IHS) at the Fort Peck Service Unit, Chief Redstone Health Center in Wolf Point, MT. This role is vital in ensuring patients receive coordinated, high-quality, and cost-effective care within the PRC program.


Position Highlights:

  • Meaningful role supporting patients with complex healthcare needs

Key Responsibilities:

  • Provide specialized case management and care coordination for patients with special healthcare needs.
  • Review and monitor outpatient consultations and referrals for medical necessity, cost-effectiveness, and appropriateness of care.
  • Collaborate with PRC staff, providers, and external facilities to ensure services align with CMS criteria.
  • Maintain and track statistical data on consults, monitor deferred referrals, and contribute to PRC continuous quality improvement initiatives.
  • Serve as a clinical resource, helping optimize patient outcomes and streamline referral processes.

Qualifications:

  • Graduate of an accredited Associate (ADN) or Baccalaureate (BSN) program in Nursing (recognized by the U.S. Department of Education).
  • Active, unrestricted RN license in any U.S. state or territory.
  • Minimum of 3 years of RN experience, with at least 2 years in case management and PRC-related work.
  • Basic Life Support (BLS) certification (American Heart Association).
  • U.S. Citizenship required.
