CLINICAL NURSE, WOUND CARE, PRN

Job Summary

The Wound, Ostomy, and Continence Clinical Service Nurse (WOC CSN) provides care to patients with stomas, draining wounds, or incontinence, throughout the organization. This includes utilization of clinical expertise to provide preoperative and postoperative care, counseling, and teaching to patients 0-99 years of age in need of wound, ostomy, and / or continence management. The WOC CSN's primary responsibility is to educate nurses to ensure knowledge and competency in basic wound, ostomy and continence management; and works collaboratively with the interdisciplinary team in a variety of specialties in planning and coordinating the care. The WOC CSN is an integral part of multiple teams, as a clinician, educator and consultant.

Job Duties and Responsibilities

• Plan and provide professional nursing care for patients in accordance with physician orders and established policies and procedures (Direct Care)

• Identifies patient caseload through referrals and active case finding.

• Conducts complete and accurate patient assessment

• Provides specialized nursing treatment, care and education to patients with wounds

• Provides specialized nursing treatment, care, stomal siting, and education to patients undergoing urinary or stool diversion in accordance with physician/nursing orders and established policies and procedures

• Provides specialized nursing treatment, care, and education to patients with draining wounds or requiring continence management or other specialized urologic interventions (such as clean intermittent self-catheterization or continuous bladder irrigation)

• Selects, develops and/or implements evidence-based clinical interventions and systems to achieve defined patient and system outcomes

• Serves as a resource to patients and families, providing preoperative counseling and follow-up problem solving after discharge from the hospital

• Utilizes comprehensive assessment skills in the evaluation of stoma and wound care products and supplies in an effort to provide quality, cost-effective care

• Assist patients and families with the understanding of their diagnosis and the management thereof

• Identifies patient/family education needs and priorities

• Coaches patients and families to help them navigate the healthcare system

• Utilizes teaching and learning theories to assist patients and families with physical and emotional impact of body changes and chronic illness

• Provides additional information/community resources to enhance the patient's quality of life as it relates to the alterations in body image and life style

• Participates in developing, implementing and evaluating patient education resources and programs appropriate to the patient's developmental level, health literacy level, learning needs, readiness to learn, and cultural values and beliefs

• Assesses learning needs of nursing staff; participate in planning and implementing appropriate educational forums and opportunities to address critical learning needs and skills

• Assist nursing personnel in the delivery of nursing care functioning as an integral member of the multidisciplinary team

• Establishes collaborative relationships within and across departments that promote patient safety, culturally competent care, and clinical excellence

• Serves as a resource and consultant to nursing members of the healthcare community and to the larger community in areas of wound, ostomy, and continence management strategies

• Practices collegially with medical staff and other members of the healthcare team

• Participates in and supports accreditation and regulatory activities of Duke Hospital

• Facilitates the provision of clinically competent care by staff/team through education, role modeling, teambuilding, and quality monitoring

Education

• Graduate of an accredited BSN program

License

• Current unrestricted RN license issued by the NCBON

Certification

• Nationally recognized certification in Wound and Ostomy management is required

Minimum Qualifications:

Work requires graduation from an accredited BSN program.
2 years of appropriate clinical experience required.
Nationally recognized certification in Wound and Ostomy management is required. CWON or CWOCN (Nationally recognized certification in Wound, Ostomy, and Continence certification, can also consider Wound Nurse certification with Ostomy certification being obtained 6 months.

Experience

• Minimum 1-year applicable RN clinical experience is required. At a manager's discretion, high performing Duke RNs with 6 months experience may be considered.

Facilitates patient flow and assists with the prioritization of work assignments. Main location will be Rex Raleigh Wound Center, but could be floated to Knightdale Wound Center. Responsibilities: 1.Maintains compliance with organizational policies and procedures. 2.Demonstrates clinical competence in patient care skills within the scope of practice for the registered nurse, to include patient assessment including assessment, measurement, and documentation of wounds, assisting with exams and procedures, administering medications, and providing patient education. Performs post-debridement measurement and documentation of procedure to ensure accurate billing. 3.Facilitates effective patient flow and assists with prioritization of work assignments. Collaborates with clinical staff for patients requiring the use of Hoyer lift and more than one staff member to complete patient treatment as ordered by provider. 4. Completes initial assessment for the Hyperbaric Oxygen Therapy (HBOT) safety questionnaire and collaborates with the HBOT tech prior to patient approval for service, to ensure a smooth treatment plan as ordered by the provider. 5. Completes nurse visits for ongoing ordered compression wraps, including an assessment, observation, and documentation of the condition of the wound. Responsible for communicating any concerning or notable changes to the provider when applicable. 6. Collaborates with insurance for provider orders for skin substitutions, home health visits, and wound vac treatments. Acts as a case manager for patients in the outpatient clinic. 7. Assists with the maintenance of adequate level of supplies and performs and documents medical equipment safety checks. 8. Participates in the cleaning and sterilization of surgical instruments in the clinic and adheres to the policies and procedures of the organization, consulting with Infection Prevention when necessary. 9. Adheres to the American Nurses association Code of Ethics. Recognizes, supports and utilizes evidence-based nursing practices. 10. Compliance with established safety and regulatory guidelines, including HIPAA, OSHA, and infection prevention policies and procedures. Participates in unit-based performance improvement activities. 11. Demonstrates good customer service skills when interacting with patients, families, and visitors. Exhibits effective and timely communication and demonstrates teamwork. 12. May serve as a Hyperbaric Technician as needed upon completion of necessary training and certification. • Prepares the hyperbaric chamber, equipment and materials as needed for patients undergoing hyperbaric oxygen therapy. Operates, maintains and ensures the safety of the hyperbaric facility and all associated systems and equipment. • Explains purpose and procedure of hyperbaric treatment to the patient. Keeps complete and accurate patient records as they pertain to treatment documentation. • Assists with transporting of patients as required. Reports treatments to be conducted and condition of patients to care team. Takes an active role in hyperbaric medicine education programs as well as orienting other care team members to hyperbaric oxygen therapy and the Hyperbaric Medicine Department. • Carries out hyperbaric oxygen therapy treatment protocols as prescribed by the Physician. Observes the patients and the hyperbaric systems during the course of hyperbaric oxygen treatment. Reports unusual occurrences to the Safety Officer and Physician and appropriately documents patients’ condition. Other Information Other information: Education Requirements: ● Diploma, associate degree (AND) or bachelor’s degree (BSN) from an accredited school of nursing. Licensure/Certification Requirements: ● Licensed to practice as a Registered Nurse in North Carolina, BLS. ● Hyperbaric Technologist Certification (CHT) certification if performing Hyperbaric Tech duties. Professional Experience Requirements: ● Previous experience in physician office, wound care, or urgent care preferred. Ability to document clinical information timely and accurately. Ability to effectively communicate information and respond to questions from patients, physicians, family members, and other staff.
