The Certified Nursing Assistant is responsible for performing various clinical and non-clinical patient care services for the Wound Care Center. Functions typically include assisting the Registered Nurse (RN) or Licensed Practical Nurse (LPN)/Licensed Vocational Nurse (LVN) in preparing patients for the examination, gathering and reporting patient data, and cleaning unit equipment and areas. These tasks are accomplished according to the policies, procedures and guidelines of the Center. All Healogics employees must perform their job responsibilities according to all Healogics policies, Hospital policies, as well as to accrediting organizations, federal and state regulation, and to the Centers for Medicare and Medicaid Services (CMS) guidelines, as applicable. Essential Functions/Responsibilities: Coordinates the delivery of the patient’s service prior to, during and after their visit/examination with the RN or LPN/LVN. This includes assisting patients with mobility, transfers, positioning, and correct application of orthosis/prosthesis, use of adaptive devices, etc. Dresses patient wounds as delegated by the RN Assist in vein clinic as assigned after Healogics Vein Clinic training completed Assists the RN or LPN/LVN during the intake/discharge process: Collecting, recording and reporting data that includes vitals, Temperature, Pulse and Respiration (TPR), Blood Pressure (BP) and blood sugar readings Taking wound photography and measurement Tracking the observed responses to treatment Recording care given as observed responses to treatment, including participation in wound care/Hyperbaric plan Reporting unusual observations or changes in patient condition/behaviour, Collecting specimens as requested Completes unit tasks such as cleaning exam areas, cleaning hyperbaric gurneys and chambers, sterile instrument processing, and other unit tasks as assigned. Performs Hyperbaric Oxygen (HBO) therapy related duties as needed, upon the completion of the required Healogics Hyperbaric training. Performs other duties as required. Every resident in the facility should have a gel cushion at minimum to their wheelchair/Geri chair. Braden assessments completed with all new admissions and re-admissions, quarterly and with any change in condition of the resident. If the Braden scores lower than a specialty mattress should be applied. At risk care plans should be placed for all resident because everyone is at risk. Rounds by UM’s/charge nurses and wound nurse to be performed to ensure that residents are positions appropriately, T&P q 2 hours and prn in bed and wheelchairs/Geri chairs, heels elevated and no skin to skin contact. 1. Consults with Nursing team concerning assessment evaluations and assist in planning an developing the skin care treatment to be performed. 2. Initiate requests for consultation or referral. 3. Forest Hill Health & Rehab is seeking an experienced and compassionate RN or LPN Wound Care Nurse to join our clinical team on an temporary basis, with the potential to transition into a Full-Time position . What You'll Do As our Interim Wound Care Nurse, you'll work closely with nursing leadership, providers, and the interdisciplinary team to oversee wound prevention and treatment throughout the facility. Responsibilities include: Assess, monitor, document, and treat acute and chronic wounds Provide care for pressure injuries, surgical wounds, skin conditions, and other wound-related needs Complete comprehensive skin and wound assessments Monitor wound progression and treatment effectiveness Ensure wound treatments and physician orders are completed accurately and timely Collaborate with physicians, nursing leadership, and the interdisciplinary team to develop individualized treatment plans Maintain thorough and accurate wound documentation Educate nursing staff, residents, and families on wound care and prevention Support pressure injury prevention initiatives throughout the facility Identify changes in condition and communicate concerns appropriately Participate in quality assurance and performance improvement initiatives Maintain compliance with state, federal, and facility requirements Serve as a knowledgeable clinical resource for nurses and CNAs What We're Looking For Current and active Virginia RN or LPN license Previous wound care experience required Skilled nursing, long-term care, rehabilitation, or post-acute care experience strongly preferred Wound care certification such as WCC or CWCN preferred, but not required Strong wound and skin assessment skills Knowledge of pressure injury prevention and wound treatment best practices Strong clinical judgment and critical-thinking abilities Excellent documentation and organizational skills Effective communication and interdisciplinary collaboration Ability to educate and support nursing team members Compassionate and resident-centered approach to care Temporary Opportunity with Full-Time Potential This position will initially be offered as an Temporary Wound Care Nurse role with the potential to transition into a Full-Time position based on facility needs, performance, and mutual fit.
