Collaborates with Medical leadership on wound care initiatives, research questions, and outcome measures to support program innovation and excellence. Develops educational modules for wound care to be used for internal and external education, including teaching tools for patients and families. Analyzes patterns of wounds across programs and hospitals, and identifies targeted PI activities to improve wound outcomes and mitigate pressure sore development. Establishes individual programs and courses directed at increasing staff knowledge, competency, and credentials in support of individual job objectives and the overall goals and mission of the organization. Coordinates competency evaluation and testing for professional staff in collaboration with clinical departments. Works in collaboration with Education department to assure coordination of all educational initiatives and activities to support the overall objectives and goals of the department and the organization. Evaluates the effectiveness of nursing and other clinical education programs. Maintains the database to track staff education, competency and credentials and utilizes the database to evaluate and plan for future educational activities and programs. Works under the direction of the Director of Education on nursing education projects and initiatives related to skin, wound and ostomy management. Collaborates with other members of the Education Department on joint educational initiatives. Performs other duties as requested. In this role, the successful candidate Coordinates the clinical care for patients at risk for or have actual impaired skin integrity, wounds, pressure injuries & ostomies. Responsible for the assessment, planning, evaluation, and execution of wound & ostomy care in the hospital. Guides the nursing staff on the appropriate products and treatments for patients with mild to complex wounds Job Responsibilities: Consults and evaluates skin/wounds for inpatients. Assists staff in providing care to patients with or at risk of developing wounds. Suggest proper support and positioning surface products. Monitors use of rental support surfaces Advises staff on available and appropriate wound care products. Evaluates patient’s progress documenting in chart and communicating to nursing staff. Provides information to staff and patients regarding management of ostomy or continent diversions. Assists patient and/or family in ostomy or continent care plan. Provide support and aid to patient and staff in plan to stabilize tube, protect skin at site and provide advice for suitable containment of drainage. Helps staff manage patients with urinary and fecal incontinence. Assess patient for causative factors; suggest referrals if indicated and aids in developing plan of care. Participate in PI not limited to Nursing Department and may include interdisciplinary projects. Maintains records and statistics and submits monthly. Participates in patient billing, coding and documentation as needed.
