Maya Homecare, a rapidly growing homecare company, is seeking a dedicated and experienced Per Diem Nurse to join our team. The ideal candidate will be a road warrior, willing to travel throughout New Jersey to provide exceptional patient care.

Responsibilities:

  • Conduct comprehensive patient assessments and reassessments.
  • Manage and track case progress, ensuring timely follow-ups and completing necessary assessments.
  • Maintain thorough documentation and stay on top of assessment dates.
  • Provide high-quality care and support to patients as needed.
  • Communicate effectively with patients, families, and the homecare team.

Requirements:

  • Valid RN license in New Jersey.
  • Reliable car and valid driver's license.
  • Willingness to travel throughout New Jersey.
  • Strong case management skills with the ability to manage multiple patients.
  • Excellent organizational skills and attention to detail.
