Acts is currently seeking qualified candidates for our Wellness Home Services Licensed Practical Nurse (LPN) position.

 

In this role, you will be responsible for delivering individualized, one-on-one care tailored to each resident’s unique needs, under the direction of a Registered Nurse (RN), where required by state guidelines. Specializes in managing chronic conditions, supporting post-operative recovery, and addressing complex medical needs with a steadfast commitment to safety, dignity, and comfort. Performs medication management, vital sign monitoring, and collaborates with multidisciplinary care teams, while providing clear education and guidance to residents and families. Committed to delivering exceptional, high-quality care that improves health outcomes, promotes independence, and enhances overall quality of life.

Requirements

The ideal candidate will meet the following requirements:

  • Active Licensed Practical Nurse (LPN) license in good standing with the applicable state Board of Nursing
  • Minimum of 1-2 years of clinical nursing experience, preferably in acute care or home health setting
  • Current CPR certification (or obtain within three months of employment)
  • If responsible for transporting a resident, must maintain a valid state driver’s license and provide proof of current automobile liability insurance

Pay Range

$23.69 - $28.75 / hour. Starting rate will vary based on skills and experience.
