Job Responsibilities:

  • Provide skilled nursing care to residents under the supervision of a Registered Nurse (RN)

  • Administer medications and treatments as prescribed by physicians

  • Conduct vital signs, monitor patient conditions, and report changes to RNs and physicians

  • Assist with daily living activities, such as bathing, dressing, and grooming

  • Participate in care conferences and develop individualized care plans

  • Collaborate with interdisciplinary teams to ensure comprehensive care

  • Maintain accurate and detailed records of patient care and treatment

Requirements:

  • Current LVN license in the state of CA

  • Minimum 1 years of experience in a long-term care setting

  • Ability to lift, push, and pull 50 pounds with assistance

  • Excellent communication and interpersonal skills

  • Strong observation and problem-solving skills

  • Ability to work effectively in a fast-paced environment
