Must have knowledge of practices and procedure of caregiving care sufficient enough to follow accepted practices. Responsibilities of a QMAP : Administer medication to identified residents according to State Medical Administration regulations. Complete medication administration documentation as outlined in policies and procedures. Order medications and supplies as needed. Report problems or errors to Clinical Wellness Manager. Accurately record and report incidents, errors, or accidents in appropriate documents on the eMAR system. Observe and report changes in the resident’s physical condition and cognitive/emotional status Clinical Wellness Manager as needed. Document changes in progress notes as outlined in procedures. Take resident vital signs as directed by Clinical Wellness Manager. Assist residents, as necessary, with Activities of Daily Living (ADL) as outlined in their Board and Care Plan (bathing, dressing, etc.) Respond to and act appropriately in emergency or disaster situations. Plan and complete duties with minimal direction from the supervisor. Qualifications of a QMAP : High School Diploma or equivalent. QMAP or MedTech certification is required. We are seeking a registered nurse embedded in our partner assisted living communities (ALFs/RCFEs) as the familiar face residents, families, and facility staff see every week. RN positions primarily focus their workload on wound care, start of care assessments, and providing clinical knowledge to patients and families This is not a desk job and not reactive case management. Working with other clinical teams, from therapists to physicians, your LVN, and closely with our scheduling coordinator. Job Duties Conduct clinical rounds alongside facility nursing staff with Start of Care Visits, Wound Care Visits, or Providing Clinical Knowledge to the patient and families Ensure home health care plans align with facility care plans Lead family engagement conversations for high-eligibility residents while residents are still stable and able to participate in their own care decisions A healthy balance of your visits and submitting documentation in a timely manner is required for this position Regularly checking in with your scheduling coordinator is expected Expectations/Mandatory Attending monthly all-staff meetings either in person or via Zoom Acknowledging any updates or messages from the admin team in a timely manner Excellent Coordination and Communication Skills A skilled RN demonstrating HIPAA compliance Reliable vehicle Experience Active, unrestricted California RN license (required) Current BLS/CPR certification (required) Experience in hospice, home health, assisted living/RCFE, skilled nursing, or geriatric care is strongly preferred Working knowledge of FAST and PPS scales, or willingness to be trained Strong documentation skills and EHR proficiency; solid command of HIPAA requirements Exceptional relationship skills: You'll be the trusted, consistent presence for residents, families, facility nurses, and physicians Comfort with autonomy and time management across multiple assigned communities Valid California driver's license, current auto insurance, and reliable transportation Pay: $50.00 - $60.00 per hour Benefits: 401(k) Health insurance Mileage reimbursement Paid time off Experience: Home care: 2 years (Required) License/Certification: RN License (Required) Willingness to travel: 100% (Preferred) Work Location: On the road
