Overview
The Licensed Practical Nurse works under the supervision of the Care Manager to assist the Physicians/Registered Nurses in the care of clients in their homes by provision of comprehensive skilled nursing services directed toward maximum rehabilitation to achieve the established goals and objectives identified. Has working knowledge of Medicare, Medicaid and insurance requirements, agency Policies and procedures. Demonstrate the knowledge and skills necessary to provide age appropriate care. Ability to assess data reflective of the client's status and current requirements. Able to provide care in accordance to departmental policy and procedure guides. The LPN may be required to work every other weekend depending upon client census. Supplements the nursing care needs of the client as provided by the Registered Nurse.

What you will do

  • Assists the physicians/registered nurses in the care of clients in their homes through provision of comprehensive skilled nursing services directed toward maximum rehabilitation to achieve established goals and objectives identified.
  • Provides comprehensive skilled nursing services to assigned patients in case load. Remains fully cognizant of Medicare, Medicaid, and Commercial insurance requirements, along with scope of service.
  • Implements and evaluates plan of care, treatments and procedures that contribute to the physiological and psychosocial outcomes of the patient. Collaborates with the primary RN in providing quality patient care needs as established in the Plan of Care. Provides services according to agency policies and supplements the nursing care needs of the client as provided by the RN. Assists the RN or physician in performing specialized procedures and duties.
  • Builds trusting collaborative relationships with a multi-disciplinary team to support patient care needs. Actively participates in multi-disciplinary team meetings as assigned.
  • Prepares clinical and progress notes documenting outcomes of interventions. Accurately completes all required clinical documentation consistently per RHC standards which includes completing and "locking" patients notes and verbal orders within 24 hours.
