Community Health Worker - CNA Career Advancement Opportunity (Full-Time Project)

Support Patients Beyond the Bedside

Ideal for experienced CNAs or direct care professionals ready to expand into community health and care coordination.

Full-time hours required for approximately three months with the opportunity to transition to the Elderwood Network at project completion.

If you are a Certified Nursing Assistant (CNA) or Home Health Aide (HHA) looking to expand your impact beyond traditional bedside care, we encourage you to apply and grow your career with Elderwood. As a Community Health Worker, you will support individuals with complex health needs by connecting them to resources, identifying barriers to care, and helping ensure they receive the services they need to remain safe and stable in their homes and communities.

This is a non-clinical, patient-facing role that combines community visits, care coordination support, and quality initiatives to improve outcomes for vulnerable populations.

If you are compassionate, observant, and passionate about helping medically complex individuals navigate the healthcare system, we encourage you to apply.

Special Project Opportunity

  • This role supports up to a 3-month full-time community health project through Elderwood IPA.
  • After the project period, employees may transition into per diem CNA opportunities through Elderwood Staffing Solutions, Elderwood’s internal float pool serving facilities throughout Western New York, with the ability to pick up flexible hours ranging from occasional shifts to part-time or full-time schedules. This is not required.
  • This opportunity allows CNAs to gain community health experience while maintaining ongoing employment opportunities within the Elderwood organization.

Why This Role is a Great Fit for CNAs:

  • Many Community Health Workers come from CNA, HHA, direct care, or patient support backgrounds.
  • Your experience observing patient needs, supporting vulnerable individuals, and communicating with care teams translates directly to this role.
  • This position allows you to continue helping patients while expanding into care coordination and community health.

Skills That Lead to Success

  • Strong communication and relationship-building skills
  • Ability to recognize patient needs and escalate concerns appropriately
  • Comfort working independently in community settings
  • Proficiency with EMR systems and basic computer tools
  • Passion for improving care for vulnerable populations

If you're a CNA or HHA ready to expand your impact and gain valuable community health experience, we invite you to apply and grow your career within the Elderwood network.
