Key Responsibilities:

· Administer medications to clients via various methods, including oral and injection, in accordance with service line requirements.

· Perform thorough nursing assessments, including physical and mental health evaluations.

· Accurately document medication administration and nursing assessments, adhering to service line standards.

· Conduct medication administration reviews and ensure house stock of medications is monitored.

· Communicate effectively with clients regarding medication inquiries and manage refill requests.

· Verify client data and medical history, and obtain necessary physical preliminary requirements.

· Complete all required documentation in a timely manner, including safety plans and nursing assessments.

Education, Experience, and/or Credential Qualifications:

· Current licensure as a Licensed Practical Nurse (LPN) in the state of practice.

· Graduation from an accredited practical nurse program.

· Successful completion of background check including criminal record, driving record, abuse/neglect, and fingerprint check.

· Current driver’s license, acceptable driving record, and current auto insurance.
