Overview of Position:

The advanced nurse practitioner
will assist ­­­­­a physician in the global management of patient care in the
ambulatory and acute settings.; The advanced
nurse practitioner role will include providing diagnostic, therapeutic, surgical
and preventative care, under the supervision of a physician in one of the following
specialties: Cardiology, Gastroenterology, Hematology/Oncology, Neurology,
Pulmonary &; Critical Care Medicine, Obstetrics and Gynecology, Physical
Medicine and Rehabilitation &; Pain Management, Psychiatry, Radiology, Urology,
Endocrinology, Hospice &; Palliative Care, Infectious Disease, Sports
Medicine, Rheumatology, Sleep Medicine, Nephrology.

The nurse practitioner will be responsible for
assessing, planning, implementing, evaluating, and documenting patient care, frequently
advising patients on health maintenance and disease prevention or providing
case management.;

ESSENTIAL DUTIES

1.    Conducts thorough medical histories, performs complete physical examinations (where indicated), initiates appropriate lab, radiology tests or other specialized tests required for evaluation of illness, and scrutinizes lab data on new and former clinic patients and hospital admissions to establish a record of the patient's current health status and to develop a working diagnosis and treatment plan.
2.    Must be able to work with a collaborating physician.
3.    Performs and interprets, standard laboratory, radiologic, cardiographic and other routine diagnostic procedures used to identify pathophysiologic processes as privileged by Genesis Credentialing Board
4.    Performs routine procedures such as injections, immunizations, suturing and wound care, managing simple conditions produced by infection or trauma, assisting in the management of more complex illness and injury as privileged by the Genesis Credentialing Board.
5.    May prescribe and monitor medications as allowed by prescriptive authority through applicable state Board of Nursing/Medicine and Genesis Credentialing Board. 
6.    May need to take call and work some off-hour shifts as needed.
7.    Perform in-hospital consultations. Facilitates appropriate length of stay, discharge planning, and compliance with regulatory standards for inpatient management. 
8.    Ensures continuity of care by serving as a liaison between patient and other members of the multi-disciplinary care team or with other specialty areas as necessary
9.    Participates in quality monitoring thru the review of records and treatment plans for patient outcomes periodically to assure quality care
10.    Discharge planning – Dictates discharge summaries, writes prescriptions, explains discharge instructions and confirms follow-up appointments.
11.    Other duties as assigned.


QUALIFICATIONS

1.    APRN: EDUCATION: Graduate of an accredited APRN training program. 
2.    Current Ohio RN and APRN licensure 
3.    Must have a current certificate of authority and certificate to prescribe (or eligible) by the Ohio Board of Nursing. 
4.    Current DEA registration (for Interventional Radiology only)
5.    Acute Care, Surgical, Critical Care, or Trauma Experience Preferred. 
6.    Required certifications:  BLS to be obtained within six (6) months of hire. 
NOTE:  Additional certifications, such as ACLS, may be required based on specialty area.

Organization: VPEC Job Summary: The specialty nurse practitioner (NP) or Physician Assistant (PA in the ambulatory care setting works in collaboration with a multidisciplinary health care team. The specialty NP/PA is generally service/physician aligned, managing a distinct patient population in the outpatient setting. The NP/PA functioning primarily in a clinic setting may be required to perform some coordination of inpatient care and facilitate discharges/outpatient follow up. The specialty clinic NP/PA has a strong foundation in evidenced based practice/education and is responsible for ensuring adherence to clinical pathways/protocols and ensures an environment of safe quality care. The NP/PA demonstrates an advanced level of medical and nursing knowledge, clinical/technical competence, sound clinical judgment, professional communication skills, timely and compliant documentation, and responsible for professional development and competency validation. . Position Hours: 8:00am - 6:00pm (10 Hour Shifts) No Weekends or Holidays Department Summary: VPEC has two locations: The Vanderbilt Clinic (TVC) and One Hundred Oaks (OHO), both are fully staffed clinics with 11 Nurse practitioners, 5 registered nurses and multiple MAs and PSRs. The staff at either location performs: vitals, images, sleep apnea screening and a complete review of systems and history and physical on patients undergoing elective surgeries. The nurse practitioners performs these visits in person, while the registered nurses use a virtual phone call model. We can also arrange for lab work and x-rays to be done at the time of the VPEC visit or through a local primary care doctor for our phone call patients. We gather any outside testing and can order electrocardiograms, cardiac consults, stress tests and echocardiograms if felt needed by the attending anesthesiologist. Patients are usually referred to the clinic by one of our surgeons for medical optimization prior to the day of surgery. This process allows our team of Nurse Practitioners to ensure that preoperative patients are properly evaluated, educated and that all necessary testing and patient information are in hand before procedures. This is a free evaluation to assess the patients overall risk for surgery. This better ensures patient safety and helps prevent surgical delays and cancellations on the day of surgery. Once the patient evaluation is completed at VPEC, it becomes part of the patients electronic medical record and is accessible by any Vanderbilt medical provider. VPEC has received several significant industry accolades, including the 2014 Top Performer Award and the coveted 5-Star Award for Overall Quality of Care. This means a score in the 90th 99th percentile based on results and as compared to similar centers nationwide. The award was given to both VPEC clinic sites. VPEC leadership is frequently consulted by other hospitals wanting to improve their preoperative processes. Nurse Practitioner (FNP, ANP, AGPCNP, AGACNP) or Physician Assistant (PA) Prior APP Experience Strongly Preferred KEY RESPONSIBILITIES Assessment of Health Status Performs and documents in the medical record a complete history and physical examination for acute and complex chronically ill patients. Orders and collects data using appropriate assessment techniques, relevant supporting diagnostic information and diagnostic procedures where indicated. The nurse practitioner may serve as the first responder for both nursing and house staff in the event of patient status changes, orders, and in situations requiring procedural and resuscitative interventions. Assesses for risks associated with the care of the acute and complex chronically ill patient including medication side effects, immobility, impaired nutrition, fluid and electrolyte imbalance, immunocompetence, invasive interventions and diagnostic procedures. Coordinates daily clinic schedule with the attending physician team, presents patient report, collaborates in plan of care, and follows assigned patient population. Diagnosis Collaborates with interdisciplinary health care team in making diagnoses of acute and complex chronic conditions. Manages diagnostic tests through ordering, interpretation, performance, and supervision. Formulates differential diagnoses by priority. Diagnoses complications and orders appropriate interventions. Formulates Plan of Care Identifies expected outcomes from diagnosis, and formulates and documents a plan of care to address complex acute and chronic health care needs. Utilizes evidenced based practice guidelines or protocols in an individualized, dynamic plan of care that can be applied across the continuum. Implements and modifies plan of care Prescribes diagnostic strategies and therapeutic interventions both pharmacologic, non-pharmacologic, and surgical, needed to achieve expected outcomes. Manages further diagnostic tests through ordering, interpretation, performance, and supervision. May perform advanced procedures consistent with privileges and competency validation, including minor suturing, complex wound management, line and tube placements and removals. Communication and Collaboration Maintains ongoing communication and collaboration with interdisciplinary health care team. Facilitates and communicates with patient, family and staff to promote continuity of care across the outpatient continuum of care. Documentation Documents/dictates key components of patient's progress via daily progress note, transfer, and discharge summary, and/or clinic note where applicable. (h&p, daily progress notes, plan of care, problem lists, procedure notes, acute event note, discharge summaries in medical record per specific patient unit or service, death summary) Documentation is timely, meets critical care compliance standards and captures patient acuity.
