Facilitates discharge of patients, completes thorough EPIC Discharge Navigator documentation, providing education on medications, discharge teaching, completing the AVS and reviewing this with the patient/significant other, and facilitating any nursing home transfers. 5. Facilitates the transfer of patients to the nursing unit, completed thorough EPIC Transfer-Arrival Navigator documentation, providing all patient teaching elements as described above. 6. Provides detailed handoff report to the receiving nurse and ensures needs are addressed prior to leaving the room. 7. Participates in MDR and /or staff meetings to gain guidance if priority of discharges/admissions or both. Works closely throughout the day with the charge nurse, keeping informed of any changes. 8.
