Job Overview
Responsible for implementing and adhering to client care plans and physician orders, including developing plans of care, overseeing and training caregivers, and administering medications, treatments, and procedures. Monitors client conditions, reports changes to management, physicians, and families, and adjusts care plans as needed. Completes assessments, submits timely clinical documentation, and collaborates with scheduling to ensure client needs are met.
Job Duties
- Conducts clinical assessments and reassessments
- Supervises and supports caregiving staff, including client/caregiver introductions and ongoing guidance
- Manages medications and monitors conditions such as anticoagulation, diabetes, blood pressure, oxygen levels, wounds, and ostomies
- Provides Alzheimer’s and Dementia care expertise
- Ensures a safe, clean, and healthy environment for clients and families
- Observes and reports changes in clients’ physical, mental, or emotional condition to Clinical Director
- Maintains professional communication with clients, families, and co-workers
- Participates in staff meetings, in-service, and continuing education programs
- Completes timely documentation of services delivered and follows office policies
- Uses equipment safely and maintains client confidentiality
- Performs other related duties as assigned
Qualifications
- Current and unrestricted MA Registered Nurse (RN) license
- Minimum 2 years home care experience; case management and hospice experience preferred
- Current CPR certification
- Strong organizational, communication, and computer skills, including internet proficiency
- Reliable, punctual, and professional attitude
- Criminal background check
- Motor vehicle driving record
- Professional and personal references
- Random drug and alcohol testing
- Reliable transportation and valid MA driver’s license
- Able to work independently in clients’ homes while maintaining positive relationships with staff, caregivers, clients, and families
- Committed to following Comfort Keepers policies and procedures
- Demonstrates honesty, integrity, compassion, and a genuine concern for helping people
