Summary/Objective
This position is responsible for the overall management of incidents that occur within the facility, with focus on the review, investigation, analysis, tracking, trending, and reporting, when applicable, of data from reported incidents/variations in the standard of care, as well as continually evaluates facility processes for gaps/opportunities for improvement so that appropriate actions are taken to reduce incidents of patient harm.

- Responsible for reviewing, investigating, and overseeing activities related to reported incidents/variances (including, but not limited to, communicating/escalating potential or actual patient safety events/risks to Administration when indicated; initial event investigation & notification/scheduling of managers for review & analysis of events; as well as ensuring complete documentation of activities associated with the incidents).
- Oversees causal analyses, corrective actions, reporting, and tracking of activities associated with incidents, working collaboratively with department directors and other Quality team members for the identification and resolution of gaps in processes/opportunities for improvement and review of care provided to ensure the standards met.
- Responsible for the committees (Incidents Management and Variance Committees) that oversee the activities related to reported incidents, with follow-through on corrective action plans to ensure appropriate & effective actions are taken, as well as documentation of the activities associated with the incidents.
- Responsible for producing regular reports on trending & analyzing of incident related data with a focus on reduction in patient harm and reporting to key stakeholders, in a clear and concise format.
- Provides regular education to staff on trending issues, results of investigations/feedback on variances, etc.
- Participates in hospital processes and programs (i.e. CERT Rounding, Center of Excellence or Disease Specific Care Committees) and medical staff (i.e. Section Meetings) meetings, as needed. May be required to present data, if indicated.
Required Education and Experience
RN or LVN: Minimum 3-5 years experience in health care, with knowledge of medical terminology, basic anatomy, physiology, and disease processes. Must be able to lead teams, navigate a medical record, prepare basic reports, and have basic knowledge of Microsoft Word and Excel programs.
Required License/Certifications
Current licensure in Texas as a Licensed Vocational Nurse or higher or a recognized compact state license.
