JOB SUMMARY Provides residents with daily nursing care in accordance with current federal, state and local standards, guidelines and regulations, facility policies and as may be directed by the Charge Nurse, Supervisor, Assistant Director of Nursing, Director of Nursing or Administrator to ensure that the highest degree of quality care is maintained at all times. SUPPORT DUTIES Commit to continued growth as a knowledgeable and experienced healthcare provider in the area of post-acute care, custodial care, and Alzheimer’s dementia and other diseases that cause dementia. Commit to obtaining at least 12 hours of continual education annually for self, including the required 16 hours of orientation training and any other ongoing dementia care specific trainings for direct care staff working on a memory care unit. Become knowledgeable of each individual resident’s background, life history, disease, medical and behavioral care needs, and preferences in detail. Utilize any communication devices for residents, such as language services, communication boards, assistive devices, etc. Accept education for utilizing these mechanisms to provide ample communication. Assist in providing a therapeutic, person-centered care that constantly strives to enhance the quality of life of each resident. Encourage family involvement through volunteering and visitation. Provide a clean, safe, non-hazardous unit for the residents and staff. Assist in providing an individualized symptom management tracking system as needed per resident, including development and maintenance of individualized, non-pharmacological interventions implemented. Complete all required documentation. Maintain all program policies and procedures and state and federal regulations. Provide feedback to nurses for monitoring achievement of restorative goals through a team approach. Be flexible to meet the change in residents’ needs. ESSENTIAL DUTIES AND RESPONSIBILITIES: Measure residents' temperature, pulse, respirations, blood pressure, weight, intake, output and food consumption and document in the official medical record. Keep residents' water pitchers clean and filled with fresh water on each shift and within easy reach of the resident. Prepare residents for meals (i.e., take to bathroom, wash hands, comb hair, raise bed, position tables, place clothing protector, take to/from dining room, etc.). Record the resident's food/fluid intake, report changes in the resident's eating habits. Assist residents in accordance to their needs ranging from minimal assistance to total dependent care on activities of daily living (ADLs). Care for residents with mental and psychosocial disorders as well as residents with history of trauma and/or post-traumatic stress disorder and who have been identified in the facility assessment. Assist in preparing the resident for exams, test, and procedures; provide support or escort residents to appointments. Assist in transporting residents to/from appointments, activity and social programs, etc., as necessary. Assist residents with bowel and bladder functions (i.e., take to bathroom, offer bed pan/urinal, protable commode, etc.). Assist non-diabetic residents with nail care including clipping, trimming, cleaning the fingers, fingernails, and toenails. Assist with lifting, turning, moving, positioning, and transporting residents into and out of beds, chairs, bathtubs, wheelchairs, lifts, etc. Change bed linens in residents' rooms. Check each resident routinely to ensure that his/her personal care needs are being met in accordance with his/her wishes. Collect specimens as instructed (i.e., urine, sputum, stools, etc.). Maintain intake and output records as instructed. Observe and report the presence of pressure areas and skin breakdowns to prevent pressure injuries. Perform restorative and rehabilitative procedures as instructed in accordance with the individualized plan of care. Provide non-pharmacological interventions for pain in accordance with the plan of care. Provide daily range of motion (ROM) exercises; record data as instructed. Assist nurses with admission transfer and discharge functions. Greet residents and escort them to their room; provide new resident orientation to the unit/facility. Keep the call light within easy reach of the residents. Follow established infection prevention and control procedures. QUALIFICATIONS: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements below are representative of the knowledge, skill and/or ability required. Must be a licensed Certified Nursing Assistant (CNA) in accordance with laws of the state. Experience in Long Term Care industry, preferred. CPR certification, preferred. PHYSICAL and SENSORY REQUIREMENTS: The physical and sensory requirements are representative of those that must be met by an employee to successfully perform the essential functions of the job.
