Overview The Observation Unit is specialized and designed for efficient, ongoing medical treatment, assessment, and reassessment of patients before the appropriate decision can be made to either discharge or admit. You will care for a wide variety of patients with chest pain, A-fib, cellulitis, syncope, asthma, and congestive heart failure, trauma, musculoskeletal pain, stroke and TIA, among other diagnosis. This is a fast paced service. Patients have a short length of stay and the unit admits and discharges patients 24/7. Case Management Coordinator

Overview About the Role: The primary purpose of the Case Management Coordinator position is to coordinate the patient’s progress through the system. He/she collaborates with physicians and clinical staff to ensure resource utilization is appropriate to the patient’s clinical and psychosocial needs and that it remains within covered benefits. The Case Management Coordinator seeks creative solutions to process and system issues, which create a barrier to the delivery of medically appropriate, cost-effective care. He/she is responsible to ensure compliance with regulatory standards related to level of care determinations, utilization and discharge planning. The Case Management Coordinator performs transitions of care and discharge planning, and supports the mission of University Hospital. Overview About the Role The Advanced Practice Nurse (APN) is a skilled health provider who through a written collaborative practice agreement with a physician is credentialed through the Medical Staff Office. As a member of the Medical Staff the APN demonstrates standards of professional practice that includes ethical decision making, job activities related to quality assessment/improvement, self-evaluation, leadership, and research. The APN is self-directed and purposeful in seeking necessary knowledge and skills to develop and maintain competency. The APN in the Surgery Practices utilizes critical judgment in the performance of comprehensive health assessments, differential diagnosis, and the prescribing of pharmacologic and non-pharmacologic treatments in the direct management of patients diagnosed with a variety of surgical conditions/diseases. The APN practice will promote wellness through conducting research, providing education and works autonomously as well as in interdisciplinary teams. Responsibilities What You’ll Do Collects comprehensive patient health data/history including past and present medical, surgical, family, and social history. Diagnostic tests and procedures relevant to the patient’s status are collected, interpreted, and documented at the Electronic Medical Record (EMR). Completes the medication reconciliation process by documenting the current list of medications (including over the counter and nutritional supplements) taken by patient at each visit. Performs and documents an appropriate comprehensive physical assessment of patients which will include a review of systems, evaluation of manifestations and toxicities related to medical conditions and anticoagulation management. In collaboration with the physician, critically analyzes the assessment data/diagnostic tests and determines a diagnosis.
