Job Summary:

Work Environment:

Work is performed in a skilled nursing facility with potential exposure to infectious diseases and hazardous materials. Appropriate personal protective equipment (PPE) is provided.

Responsibilities:

Patient Care: Provide direct nursing care to residents, including administering medications, wound care, and treatments as prescribed by the physician.
Monitoring and Assessment: Monitor residents’ vital signs, document their progress, and report any significant changes to the Charge Nurse or Director of Nursing.
Care Planning: Assist in the development and implementation of individualized care plans, ensuring they are tailored to meet each resident’s needs.
Communication: Maintain clear and accurate documentation of patient care, communicate effectively with residents, families, and the healthcare team.
Compliance: Adhere to all facility policies, procedures, and regulatory requirements, including infection control and safety protocols.
Support: Provide emotional support and education to residents and their families, helping them understand care plans and health conditions.
Team Collaboration: Work collaboratively with other healthcare professionals, including RNs, CNAs, physicians, and therapists, to deliver coordinated and comprehensive care.

Qualifications:

Licensure: Valid LPN/LVN license in Oklahoma
Education: Graduation from an accredited LPN/LVN program.
Experience: Previous experience in a skilled nursing facility or similar healthcare setting preferred. Certifications: Current CPR/BLS certification required. Additional certifications may be required based on state regulations.
Communications: Excellent verbal and written communication skills. Ability to work collaboratively with a diverse team of healthcare professionals and build positive relationships with residents and their families.
Physical Requirements: Ability to lift and move patients, perform physical assessments, and stand for extended periods.

Norman Regional Health System and Oceans Healthcare have partnered to expand access to behavioral health services in south central Oklahoma through construction of a new, state-of-the-art behavioral hospital. Behavioral Health Center at Porter Health Village, a 48-bed facility, will more that double Norman Regional Health System’s inpatient capacity for adult and senior behavioral health patients and add new services, including an intensive outpatient program and a dedicated geriatric behavioral health unit. The freestanding hospital will be located on the Norman Regional Porter campus and replace the existing 20-bed behavioral health unit. The Licensed Practical Nurse's primary role is to provide nursing care for patients who are experiencing common, well-defined health problems. The LPN functions as a member of the multidisciplinary health care team, and utilizes the nursing process in implementation of patient's care. Those services are performed under the supervision of a registered nurse but utilize all nursing skills to make safe, appropriate and independent decisions within the scope of the LPN nursing practice.
