Position Summary

We are seeking a compassionate and experienced Registered Nurse (RN) Hospice Case Manager to join our interdisciplinary hospice team serving the Lehigh Valley area. This role focuses on providing high-quality, patient-centered care to individuals and their families during end-of-life, while supporting comfort, dignity, and quality of life.

Key Responsibilities

  • Manage and coordinate care for an assigned hospice caseload

  • Perform comprehensive patient assessments and develop individualized plans of care

  • Provide skilled nursing visits in patients’ homes and other care settings

  • Collaborate with physicians, social workers, chaplains, aides, and other team members

  • Educate patients and families on disease progression, symptom management, and medications

  • Monitor patient status and adjust care plans as needed

  • Ensure accurate, timely clinical documentation in accordance with hospice regulations

  • Participate in interdisciplinary team meetings and care planning

  • Serve as a patient and family advocate throughout the hospice journey

Qualifications

  • Current Pennsylvania RN license (required)

  • Minimum 1 year of nursing experience (hospice, home health, or case management preferred)

  • Strong clinical, organizational, and communication skills

  • Compassionate, patient-focused approach to care

  • Valid driver’s license with reliable transportation
