Mercy Home Health, a member of Trinity Health at Home, is seeking a dedicated Full-time Registered Nurse for our Palliative Care program. This position is based out of Newtown Square and supports Chester and Delaware Counties.

Key Responsibilities:

  • Provide comprehensive care to patients and families in the palliative care program.
  • Offer clinical consultation in palliative care.
  • Develop, coordinate, and evaluate palliative and end-of-life care programs.

Qualifications:

  • Education: BSN required.
  • Licensure: Current PA RN License and CPR Certification.
  • Certifications: Hospice/Palliative Care Certification required.
  • Experience: Home care experience, strong clinical knowledge base, and hospice experience preferred.
  • Skills: Strong interpersonal, organizational, problem-solving, critical thinking, and priority-setting skills. Ability to work independently and as part of a team. Proficient in computer skills.

    Minimum Qualifications:

    • Graduate of an approved nursing education program.
    • Licensure as a Registered Nurse in the state of PA.
    • Palliative Care Certification is required
    • One (1) year of experience as a professional care nurse in home care or hospice care is a plus.
    • Must have a current driver’s license and reliable transportation.

A Brief Overview Children's Hospital of Philadelphia is seeking a Primary Care Nurse for their West Grove Primary Care office. This position will be Monday-Friday and will require one evening shift per week and a rotating Saturday shift. May need to travel to other Primary Care sites under manager's cluster. The shift and location and will be based off of the operational need. Pediatric experience is preferred. The Primary Care Nurse is a clinically competent practitioner of pediatric, family centered nursing. The nurse at this level provides safe nursing care to the general unit population with minimal guidance, and bases practice on identified principles and standards of care. The nurse must demonstrate competence in caring for patients with moderate to complex health issues and assist the Site Manager with administrative issues which affect the clinical practices within the office as well as comply with the administrative policies and clinical protocols as outlined in policy and procedure manual. Administratively responsible to the Site Manager and Ambulatory Nursing Director for nursing practice issues. Must be willing to rotate to other Care Network sites within the designated cluster. What you will do Follows Nursing Process to deliver safe care: assessment, planning, implementation, and evaluation Synthesizes patient assessment data and recognizes complex situations prior to communicating with care team.
