The Registered Nurse (RN) is responsible for providing direct guest/resident care and supervision and oversight of the guest/resident care by the licensed and non-licensed team members in the skilled nursing center. Essential Duties

Clinical Care

  • Assists with the development and leadership of the Designated Nursing Assistants (DNA)/ Certified Nursing Assistants (C.N.A); follows Resident Centered Care model.
  • Assists with case management of both Medicare and non-Medicare guests/residents as needed.
  • Participates in the Care Planning meetings.
  • Assists with providing clinical care and not limited to: urinary cauterization, tube feedings, applying/changing dressings, Accu-checks, medication management/administration, admission, transfer and discharge of guests/residents.
  • Ensures interdisciplinary notes reflect the care provided through daily random audits.
  • Completes timely and accurate documentation of guests/residents charts.
  • Completes Incident Reports and reports to Director of Nursing Services (DNS).
  • Reviews, updates and documents communication regarding residents to learn and share pertinent resident and community information.
  • Conducts effective visits/rounds at a minimum twice a day; conducts visits/rounds with physicians as necessary as assigned by DNS.
  • Research and documents all incidents of abuse, neglect and misappropriation of funds as assigned by DNS or Executive Director, Skilled Nursing Administrator.
  • Participates actively in the Clinical IDT Meetings and in the Daily Prospective Payment System (PPS) meetings to obtain information on admissions and discharges for the day / week.
  • Assists in monitoring medication records and administration.
  • Assists in receiving lab, x-ray and other test results and communicates appropriately.

Quality Assurance, Regulatory Compliance

  • Strives for excellent quality care and service delivery as measured in the 5 Star and Quality Measures process in demonstration and knowledge of standard practices and excellent resident care and services.

  • Monitors for resident decline /improvement in condition.
  • Refers resident/guests to restorative nursing as indicated.

Clinical Systems

  • Demonstrates Sunrise practices and guidelines for care and services surrounding guest/resident’s skin, nutrition, behavior, falls, bowel/bladder.
  • Assists in implementation, training and monitoring of documentation related to clinical systems weekly
  • Consults with physician as appropriate.
  • Ensures guest/resident changes, concerns and/or solutions to Director of Nursing Services as appropriate.

Infection Control

  • Understands and follows infection control practices.
  • Provides training as needed to support infection control practices.
  • Understands and practices the proper method of attending to and disposing of and the possibility of exposure to blood borne pathogens, bodily fluids, infectious waste, sharp sticks and hazardous materials in compliance with universal care precautions.
  • Reports occupational exposures to blood, body fluids, infectious materials, sharp sticks and hazardous chemicals immediately.
  • Ensures oxygen tanks are stored safely, exchanges guest/residents tanks when empty and monitors to make sure liters of oxygen are at prescribed levels.

Medication Management

  • Transcribes physician orders eMAR (Medication Administration Record / Treatment Administration Record) accurately and promptly to include initials, date and time ‘noted’ and date faxed to pharmacy.
  • Demonstrates and knowledgeable of the Six (6) Rights of Medication Pass (“Right” resident, medication, dosage, time, route, right to refuse).
  • Maintains knowledge in the appropriate medication for appropriate diagnosis for the senior population.
  • Provides leadership and follows Sunrise and nursing standards for taking orders from physicians and administering medications.
  • Ensures current guest/resident photo with dates for all guest/resident on eMAR.
  • Completes eMar audit each month.
  • Monitors the Medication Room, med charts, treatment carts for neatness, cleanliness, availability of medications and expired medications.

Guest/Resident, Family and Visitor Relations

  • Responds to guest/resident and family, visitor’s request in a timely and professional warm manner.
  • Communicates with families regarding the guest/resident medical care and seek new methods to incorporate guest/resident requests into existing nursing standards.
  • Coordinates and utilizes available health care resources.
  • Maintains confidentiality of all guest/resident information.
  • Reviews SCC dashboard 24 hour report, high priority progress notes.

Risk Management and Safety

  • Partners with community team to ensure community is in compliance with national/provincial regulations pertaining to occupational health and safety requirements and promotion of Risk Management programs and policies; adherence to safety rules and regulations.
  • Practices safety procedures at all times including Personal Protective Equipment (PPE), fire extinguishers, Safety Data Sheet (SDS) and Lockout Tagout procedures.
  • As applicable, all Sunrise team members who drive a Sunrise vehicle must sign the Driver job description and understand the key essential duties for safety and regulatory compliance.

Training, and Contributing to Team Success

  • Participates actively as a member of a team and committed to working toward team goals.

Experience and Qualifications

Provide clinical care through the direct application of the nursing process; perform and document resident assessments and progress notes, evaluate changes in care needs, complete Individualized Service Plans (ISP), provide or delegate hands-on clinical care as indicated by the plan of care, and evaluate resident outcomes. Collaborate with physicians, pharmacists, and other clinical providers to coordinate care and services for the residents. Serve as the CLIA Director as applicable for the community and according to the federal and provincial requirements. Partner with Neighborhood Coordinators to promote an integrated and collaborative approach to wellness operations and resident care. Collaborate with Sales partners to determine community capability and assess potential residents’ appropriateness for move-in. Medication Management Provide strategic leadership for resident care in the community by managing, directing, and monitoring the medication care managers, as applicable by State/Provincial law, to promote the health and wellness of the resident population. Provide oversight of the community medication management program to promote resident safety in the medication use process including onboarding, training, and performance reviews. Provide clinical training and education, as needed, to nurses, medication care managers, care managers, and others who provide resident care. Perform skill evaluation to assure clinical capability of care team members. Quality Assurance and Regulatory Compliance Track, trend, and report clinical quality data to identify risk. Participate actively as a member of the community Quality Assurance and Performance Improvement committee. Lead clinical quality and process improvement initiatives within the community to mitigate risk and improve resident care outcomes. Recruit, hire, and train clinical team members and is responsible for performance management, evaluations, and engagement. Complete direct report team member staffing and scheduling according to operational and budgetary guidelines. Partner with the community leadership team to promote resident safety and compliance with Risk Management national/provincial regulations pertaining to occupational health and safety requirements. Serve as the ICC and CLIA Director as applicable for the community. Responsible for infection control programming, including delegation of infection control preventionist, as per provincial requirements. Assure compliance with all Federal, State/Provincial, board of nursing, and other applicable regulations. Financial Management Manage the department budget to include labor/labour and other expenses and understand its impact on the community’s bottom line. Processes and submit monthly expenses and budget data in a timely manner, per Sunrise policies and internal business controls. Understand the internal costs associated with all Sunrise resident care programs. Training, Leadership and Team Member Development Partner with the Regional Director of Resident Care and/or Executive Director in the delivery and participation in Sunrise University training and self-study programs. On-board new RCD leaders and other department coordinators as needed. Develop a working knowledge of state/provincial regulations and ensure compliance through supervising and coaching team members. Complete clinical team member staffing and scheduling according to operational and budgetary guidelines when assigned to a community. Attend regular meetings; Stand Up, Cross Over, Department Head, Town Hall, QAPI, and others as directed by the Executive Director. Keep abreast of professional developments in the field by reading and attending conferences and training sessions. Field Registered Nurse - ACT (Part time )

POSITION OVERVIEW: As a member of the ACT team, the RN is responsible for providing a range of treatment to clients with a SPMI/SUD, rehabilitation and support services within the scope of the nursing practice for the clients receiving services from our ACT teams across the city. S/he is responsible for conducting initial nursing assessments and ongoing health monitoring assessments; assessing physical health needs; making appropriate referrals to community physicians as needed, providing medical case management and administration of medication in conjunction with the prescriptions/orders of psychiatrist, PNP, and medical providers. The majority of this role is conducted in the community and is not site specific. Each nurse will work with a primary program and may also work with other JB programs across the city. The RN will be supervised by the Program Director/Nursing leadership. KEY ESSENTIAL FUNCTIONS : Carry a caseload and complete initial and ongoing nursing assessments. Conduct weekly visits thereafter for clients currently residing in homeless shelters and assigned to the ACT program and/or in client’s homes or elsewhere in the community where the client is accessible. Develop a treatment plan with the team, based on clients’ needs. Ensure that any medical diagnoses are addressed and the client will be educated as to conditions and signs and symptoms. Complete nursing assessments and health monitoring assessments within appropriate timeframes. Will follow/implement all direction from Psychiatrist or PNP regarding medical treatments and medication orders. Will administer prescribed and/or routine medical treatments, support clients and staff with self-administration of medication procedures and evaluate for potential side effects or adverse reactions. Ensure that all internal policies regarding such administration are adhered to. RN will partner with our clients, in a respectful, encouraging way to promote healthy lifestyle changes within a harm reduction framework, with shared decision-making practices. Will coordinate and deliver medications to the client’s residence or shelter, be responsible for medication counts, follow-up the pharmacies (i.e. refills, insurance authorizations etc) and administering medication to clients. Make referrals, schedule appointments and accompany the client when required. Make visits to hospitalized clients to coordinate care. Provide advocacy for hospitalized clients. Assist with follow up care to promote complete recovery. Function as a liaison between external medical health care providers, shelters, other community organizations and the ACT team with regards to the client’s medical needs. Responsible for health education and communicating information regarding good health practices to the clients under RN's care. Train staff in these practices as well so as to provide continuous health support to clients. Facilitate groups. Respond to medical emergencies as appropriate. Perform all necessary charting and other documentation as required by internal practice and external regulatory bodies within a timely manner. Follow-up and monitor lab results for any indications of change for medication and toxicology; communicate such change(s) to the psychiatrist and PNP.
