POSITION SUMMARY:   The Advanced Practice Professional (APP) has coordination and clinical skills required for the comprehensive management of cardiology/electrophysiology patients, (including biological, social and psychological assessments).  The APP coordinates involvement of medical/surgical specialty teams necessary to develop a comprehensive plan for the patient, and reviews the plan of care with the other members of the clinical team, participates in directing that care, performs diagnostic/therapeutic procedures, peri-operative services, and coordinates discharge planning (as applicable).  The APP participates in the education of residents and medical/nursing students. 

Provide healthcare services consistent with state law and the requirements of the professional licensing or certification authority and/or hospital privileges, where applicable.  Such service may include but are not limited to.  

MINIMUM QUALIFICATIONS FOR NURSE PRACTITIONER:
EDUCATION, CERTIFICATION, AND/OR LICENSURE:

1.    Graduate of a CCNE accredited nursing program with Master’s Degree in Nursing
2.    National Certification by one of the recognized APRN Certification Centers
3.    Licensure, or eligibility for licensure, as a professional Registered Nurse and Advanced Practice Registered Nurse in West Virginia.
4.    American Heart Association BLS Certification required with additional Advanced Life Support training at the discretion of the 
department.  


EXPERIENCE:
1.    Cardiology/electrophysiology or acute care experience preferred.  
2.    Eligible new graduates will be considered.

PREFERRED QUALIFICATIONS:
EDUCATION, CERTIFICATION, AND/OR LICENSURE:

1.    AGACNP certification preferred.  FNPs will be considered.

1.    Bachelor’s Degree as Physician Assistant, or Master’s Degree as Physician Assistant required.

4.    Must be licensed or eligible for licensure by the WV Board of Medicine or WV Board of Osteopathic Medicine.

1.    Manages comprehensive and individualized care for cardiology/electrophysiology patients and their families in the inpatient phase of hospitalization and immediate post discharge in the ambulatory setting.


2.    Completes daily rounds; consults and/or pre-procedure/evaluations.


3.    Participates in electrophysiology conduction studies including but not limited to management/monitoring of implantable devices.  


4    Completes integrated assessments.


5.    With the collaboration of cardiology/electrophysiology faculty or by following established clinical Guidelines and Protocols, orders additional tests or consults as deemed appropriate.


6.    Performs and communicates results of diagnostic tests following established clinical guidelines and scope of practice.


7.    Consults and communicates with clinic and hospital staff regarding abnormal findings in the evaluations of patients.


8.    Performs succinct comprehensive clinical evaluations and documents findings in medical record.


9.    Develops a comprehensive individualized plan of care for cardiology/electrophysiology patients, which includes teaching, discharge planning, ongoing monitoring of implantable devices and/or necessary referral to other members of the health care team as necessary.


10.    Demonstrates expertise in caring for patients across the age continuum of their educational experience and certification.


11.    Identifies unstable or complex cardiology/electrophysiology patient care situations and facilitates appropriate intervention in the clinic and/or hospital setting.


12.    Recognizes emergency situations and initiates effective emergency care.


13.    Evaluates cardiology/electrophysiology patients and serves as a coordinator for follow up procedures and tests in the ambulatory setting (as applicable).


14.    Assists in the collection of outcome measures (as applicable).


15.    Demonstrates critical thinking and diagnostic reasoning skills in clinical decision making by the identification, evaluation and management of the individual needs of patients/families.


16.    Demonstrates the ability to utilize equipment in a safe manner.


17.    Bases decisions made/actions taken on the relevant scientific principles, established standards of care and/or research findings.


18.    Utilizes teaching/learning theory and appropriate teaching strategies in the provision of education to nursing staff, hospital staff, students, patients, families and the community.


19.    Promotes an environment that facilitates learning by responding to appropriate patient/family preferences, priorities and readiness to learn.


20.    Updates own knowledge base throughout the year.


21.    Completes all identified clinical competencies.


22.    Serves as a resource person for staff caring for the cardiology/electrophysiology patient.

24.    Participates in community programs, distribution of literature, as appropriate in area of expertise.


25.    Documents the plan of care, interventions and evaluations in a clear and concise manner for every patient visit in the patient’s medical record following approved hospital/UHA policies and compliance standards for documentation.  


26.    Collaborates with other leaders and staff to effectively achieve patient outcome-based improvements in care.


27.    Provides leadership in the development and implantation of changes in the cardiology/electrophysiology practice that positively impact patient outcomes.  


28.    Works collaboratively with medical and nursing staff to facilitate creative problem solving and provide professional support.


29.    Participates and initiates inter-disciplinary approaches to patient care, program development and education.  Serves as a professional role model.


30.    Promotes and practices innovation of the expanded role in the delivery of care to cardiology/electrophysiology patients and families throughout the inpatient hospitalization, and the ambulatory setting.


31.    Participates and provides leadership for committees and task forces.


32.    Documents hours and submits billing reports, as appropriate.  


33.    Reviews and interprets cardiology/electrophysiology data from sources such as patient registries, clinical and financial reports, length of stay, access to care, and cost per case reports and recommends/implements changes as necessary.


34.    Participates in clinical trials and research studies with the Principal Investigator.


35.    Assists cardiology/electrophysiology Faculty, Administrators and Directors in the collection of outcome measures.


36.    Other duties/projects as assigned.

Organization: VPEC Job Summary: The specialty nurse practitioner (NP) or Physician Assistant (PA in the ambulatory care setting works in collaboration with a multidisciplinary health care team. The specialty NP/PA is generally service/physician aligned, managing a distinct patient population in the outpatient setting. The NP/PA functioning primarily in a clinic setting may be required to perform some coordination of inpatient care and facilitate discharges/outpatient follow up. Position Hours: 8:00am - 6:00pm (10 Hour Shifts) No Weekends or Holidays Department Summary: VPEC has two locations: The Vanderbilt Clinic (TVC) and One Hundred Oaks (OHO), both are fully staffed clinics with 11 Nurse practitioners, 5 registered nurses and multiple MAs and PSRs. The staff at either location performs: vitals, images, sleep apnea screening and a complete review of systems and history and physical on patients undergoing elective surgeries. The nurse practitioners performs these visits in person, while the registered nurses use a virtual phone call model. We can also arrange for lab work and x-rays to be done at the time of the VPEC visit or through a local primary care doctor for our phone call patients. We gather any outside testing and can order electrocardiograms, cardiac consults, stress tests and echocardiograms if felt needed by the attending anesthesiologist. Patients are usually referred to the clinic by one of our surgeons for medical optimization prior to the day of surgery. This process allows our team of Nurse Practitioners to ensure that preoperative patients are properly evaluated, educated and that all necessary testing and patient information are in hand before procedures. This is a free evaluation to assess the patients overall risk for surgery. This better ensures patient safety and helps prevent surgical delays and cancellations on the day of surgery. Once the patient evaluation is completed at VPEC, it becomes part of the patients electronic medical record and is accessible by any Vanderbilt medical provider. VPEC has received several significant industry accolades, including the 2014 Top Performer Award and the coveted 5-Star Award for Overall Quality of Care. This means a score in the 90th 99th percentile based on results and as compared to similar centers nationwide. The award was given to both VPEC clinic sites. Breast Surgery Nurse Practitioner

Position Summary The Telfair Breast Surgery NP/PA works under the clinical supervision of the TBS Breast Specialty Surgeons and administrative supervision of the Director, Telfair Ambulatory Services. The role builds upon general nurse practitioner training and skill sets by requiring specialty focus in Breast Diseases with particular focus on high risk for breast cancers and breast cancer surgical patients, treatment management, and pre-and post-surgical follow-up. The NP/PA maintains a case load of patients and bills appropriately for services rendered. Under the direct clinical supervision of the TBS Medical Director and the partner surgeons, the TBS NP/PA will run a high risk clinic for patients qualifying for high risk for breast cancer management. Clinical support duties, under the supervision of TBS surgeons, include appropriate ordering of diagnostic work up studies per high risk management protocols, monitoring and evaluating progress of patients under surveillance, rounding on patients, first call triage, and ongoing monitoring of patients on clinical protocols by working closely with clinical research coordinator. The NP/PA is expected to bill appropriately and maintain all licensures and privileges to do so. The NP/PA is required to meet all regulatory and compliance aspects of the role and to participate in quality and revenue cycle improvement activities. Conducts comprehensive holistic wellness and illness assessments and employs evidence-based clinical practice guidelines in care of patients. Provides leadership for collaborative, evidence-based revision of diagnoses and plans of care, to improve patient outcomes. Develops evidence-based clinical interventions and systems to achieve defined patient and system outcomes. Appropriately assess and managers patients; accurately prepares procedure and diagnostic orders, monitors, and evaluates patient progress, promptly responding to changes in patient status. Works under the guidance of surgical oncologists. Routinely attends available continuing educational programs in order to maintain competencies as an oncology nurse practitioner and stays current with advancements in oncology patient management. Job Details Physician Office l Wilmington l Full-Time l First Shift Responsibilities & Requirements The APP is a provider of oncology health services who synthesizes and utilizes evidence-based practice, research, and current clinical knowledge. Working in collaboration with a multidisciplinary health care team. The APP demonstrates an advanced level of medical and/or nursing knowledge, clinical and technical competence, sound clinical judgment, professionalism as it relates to interpersonal and general communication skills, timely and complete documentation and take responsibility for ongoing professional development and competency validation. The APP role in partnership with supervising/collaborating physician(s) s includes history taking, clinical assessment, and physical examination, and order entry, interpretation of diagnostic studies, clinical diagnosis, treatment, and evaluation of care for the given population.
