Overview

We are seeking a dedicated and compassionate Registered Nurse (RN) to conduct in-home supervisory visits for EPSDT and VA clients. This role focuses on oversight, assessment, and support rather than skilled nursing tasks.

The RN will manage a caseload of approximately 50–70 clients, ensuring required face-to-face visits every 60 days in the client’s home. This position requires strong organizational skills, independent work capability, and a commitment to high-quality, patient-centered care.

Cuba, NM – 6 clients

Santa Fe, NM – 33 clients

Las Vegas, NM – 29 clients

Key Responsibilities: 

  • Conduct in-home supervisory visits with EPSDT and VA clients every 60 days
  • Ensure compliance with all federal, state, and organizational regulations and standards
  • Assess client well-being, safety, and overall care needs
  • Document visit findings accurately and in a timely manner
  • Communicate concerns, observations, and recommendations to appropriate team members
  • Collaborate with caregivers and interdisciplinary team members to ensure coordinated care
  • Maintain complete, accurate, and up-to-date client records
  • Provide education, guidance, and resources to clients and their families
  • Adhere to all company policies, procedures, and confidentiality requirements

Qualifications: 

  • Current, active Registered Nurse (RN) license in New Mexico
  • Minimum of 1 year of nursing experience (home health or community-based care preferred)
  • Strong assessment, documentation, and organizational skills
  • Excellent communication and interpersonal abilities
  • Ability to work independently and manage a flexible schedule
  • Demonstrated commitment to compassionate, patient-centered care
  • Valid driver’s license and reliable transportation
  • Must live in New Mexico

Overview The Registered Nurse will be responsible for conducting face-to-face supervisory visits with our EPSDT and VA clients.
