Overview We are seeking a dedicated and compassionate Registered Nurse (RN) to conduct in-home supervisory visits for EPSDT and VA clients. This role focuses on oversight, assessment, and support rather than skilled nursing tasks. The RN will manage a caseload of approximately 50–70 clients, ensuring required face-to-face visits every 60 days in the client’s home. This position requires strong organizational skills, independent work capability, and a commitment to high-quality, patient-centered care. Service Areas & Current Census (subject to change) Cuba, NM – 6 clients Santa Fe, NM – 33 clients Las Vegas, NM – 29 clients Key Responsibilities: Conduct in-home supervisory visits with EPSDT and VA clients every 60 days Ensure compliance with all federal, state, and organizational regulations and standards Assess client well-being, safety, and overall care needs Document visit findings accurately and in a timely manner Communicate concerns, observations, and recommendations to appropriate team members Collaborate with caregivers and interdisciplinary team members to ensure coordinated care Maintain complete, accurate, and up-to-date client records Provide education, guidance, and resources to clients and their families Adhere to all company policies, procedures, and confidentiality requirements Qualifications: Current, active Registered Nurse (RN) license in New Mexico Minimum of 1 year of nursing experience (home health or community-based care preferred) Strong assessment, documentation, and organizational skills Excellent communication and interpersonal abilities Ability to work independently and manage a flexible schedule Demonstrated commitment to compassionate, patient-centered care Valid driver’s license and reliable transportation Must live in New Mexico Compensation & Benefits: Competitive pay and comprehensive benefits package Paid mileage reimbursement Compensation for travel time and administrative time (including scheduling) Opportunities for professional development and career advancement Supportive, collaborative work environment Why Join Us? In this role, you'll support clients with daily living activities, ensuring comfort, safety, and dignity.
