Job Summary The role of the primary care pediatric Advanced Practice Provider (APP) is to provide care to children from birth through young adult with an in-depth knowledge and experience in pediatric primary health care including well child care and prevention/management of common pediatric acute illnesses and chronic conditions. This care is provided to support optimal health of children within the context of their family, community, and environmental setting. The Children's Medical Group (CMG) APP Fellowship is an intensive 6-month educational training opportunity for a licensed graduated practitioner without prior independent APP clinical experience. The fellowship is an innovative curriculum composed of in-person didactics, simulation, proctored procedures and mentored clinical rotations and is designed to the specific competencies and expectations relevant to the CMG APP provider role described above. As part of the CMG APP fellowship, the APP will obtain a certificate in lactation or mental health. Essential Functions - Perform comprehensive histories and physical exams for well and acute visits, identifies normal and abnormal findings, including physical, cognitive and psychosocial status. - Order, perform, and interpret screening and diagnostic procedures and tests, and appropriately communicates and documents information. - Develop pharmaceutical and non-pharmaceutical treatment plans. - Monitor effectiveness of plan of care.. - Educate patients on disease and treatment plans. Educates, counsels, and assists patients and families regarding current health problems, health maintenance activities and preventive behaviors. - Perform procedures as trained and appropriately credentialed. - Document within the Electronic Medical Record (EMR) and complete other necessary documents within a timely manner. - Collaborate and/or consult with physicians and other health care providers as appropriate as part of the care team. - Participates in organizational initiatives including the development, maintenance and support of the strategic direction of Children’s Wisconsin Primary Care. Actively participates in the clinical quality and safety efforts of the department or the organization. - Responsible for understanding and adhering to the Children’s Organizational Code of Ethics and for ensuring that personal actions, and the actions of employees supervised, comply with the policies, regulations and laws applicable to Children's business. Legacy Essential Functions - Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served. Must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to his/her age-specific needs, and to provide the care needed as described in the department's policies and procedures. Age-specific information is developed further in the departmental job standards. - Assumes responsibility for own education as an engaged participant within the CMG APP Fellowship. Participates in all required fellowship course, educational activities and feedback sessions. The fellow should be self-aware and take responsibility for clinical and professional growth and development. - Must be flexible with schedule based on fellowship course schedule. Education: - Master's Degree or higher in Nursing required or - Bachelor's Degree or higher in Physician Assistant Studies required Experience: - This position is geared towards a new graduate or clinician with less than 1 year of APP experience. preferred Knowledge, Skills and Abilities - Exhibits guiding behaviors that reflect Children’s values and support our mission and vision. How you make a difference The Nurse Practitioner (NP) under the supervision of the Site Medical Director and other site Physicians, offers a comprehensive range of medical services to incarcerated patients in accordance with established policy and state guidelines. Overview The Electrophysiology Physician Assistant will assist in providing support for patients and overall development of the FMC Electrophysiology program. The provider assists the supervising physician in the evaluation of new patient consultations, daily hospital rounding, coordination of care with other providers, patient education, hospital discharge, and assistance in procedures. The PA reports encounters to the supervising physician in support of providing quality, efficient, continuous and cost-effective patient care for Northern Arizona Healthcare (NAH). Responsibilities Patient Care * Collaborates with the attending provider via phone or in person to review consultations, plan of care, and patient status updates for all hospitalized patients.* Coordinates care with interventional cardiology providers and patients.* Provides direct patient care including obtaining a health history, performing a detailed physical exam, creating a comprehensive assessment of the patient, ordering and interpreting diagnostic tests, and prescribing pharmacological agents.* Updates patient history and physical in preparation for procedures. Obtains informed consent when necessary.* Enters and signs pre-procedural orders.* Communicates with cath lab and cardiology staff to ensure a timely flow of EP procedures, while taking into account staffing and patient considerations.* Completes documentation of all consultations, progress notes, and services rendered in the EMR in a timely manner.* Orders referrals and diagnostic testing as necessary.* Completes discharge planning and discharge summaries for hospitalized patients.* Performs procedures under the supervision of the attending physician within scope of physician assistant training.* Functions as a surgical first assist in the cath lab and operating room when assigned.
