Registered Nurse - RN Case Manager Overview:

  • Perform initial client assessments and Develop and initiate the plan of care
  • Initiates the plan of care and makes necessary revisions as patient's status and needs change
  • Develops a care plan that establishes goals based on nursing diagnosis and incorporates palliative nursing actions. Includes the patient and the family in the planning process.
  • Counsels the patient and family in meeting nursing and related needs.
  • Assume responsibility to coordinate patient care for assigned caseload.
  • Completes and submits accurate and relevant clinical notes regarding the patient's condition and caregiving.
  • Attends interdisciplinary meetings
  • Coordinate interdisciplinary services and obtain physician orders for appropriate care
  • Administer nursing cares and education
  • Communicate continuously with patients, families, and other care team members

Registered Nurse - RN Case Manager Qualifications:

  • Licensed as a Registered Nurse / RN in the state you work in
  • Hospice Experience required.
  • Proficiency with Electronic Medical Records (EMR) software is necessary
  • Prior Nursing experience in a TCU, Hospice, or Home Health setting
  • Valid Driver's License and reliable vehicle with current insurance
  • Excellent at planning, organizing, and communicating
  • Demonstrated ability to work autonomously

Moments Hospice is growing!!! We are currently looking for compassionate RN's to join our Team in a On-Call capacity. This position would work 21 days a month (not in a row), consist of 2 weekends a month and then the 5pm-8:30am shift on the weekdays. We have a Triage Team, so on call would only entail times that we need to send a RN out to see Patients/Families. As a Hospice Registered Nurse, you will: Provide skilled professional nursing care to patient/family/caregivers Oversee and provide primary care, including all clinical care, palliative care, and symptom control and identify other physical, psychological, social and spiritual needs for assigned patients. Regularly assess the patient for nursing needs. Clinical Nurse Liaison

Our Clinical Liaison professionals utilize their clinical knowledge and assessment skills in the acute care setting to provide education and create awareness of hospice as an integral part of the continuum of care. As part of the Business Development Team, the Clinical Nurse Liaison collaborates with Physicians, Case Managers, Discharge Planners, Social Workers, Directors of Nursing and other clinical partners to advocate for utilization of hospice to ensure support for patients and their families as they near end of life. Overview The Clinical Liaison, Registered Nurse (RN) will act as liaison between the contracted hospitals and the hospice team, make inpatient visits to hospice patients in contracted facilities, act as consultant for pain and symptom management, provide in-service education to providers, perform pre-admission evaluations for appropriateness for hospice services, initiates the admission process by educating and informing patient/family of hospice and it's services. For Patient's/Families electing hospice, you will also help to complete informed consents followed by providing a nursing assessment, initial plan of care and intervention(s), which optimize patient and family directed comfort and quality of life and coordinates discharge planning.
