LPN - LICENSED PRACTICAL NURSE – 3500 – Wound Clinic SUMMARY Provides nursing services to the patients and families, assists physicians with patient care in conformance with the Licensed Practical Nurse Act. To perform all clinical functions in Wound Clinic. Assisting the physician with treatments, obtain specimens from patients and process properly, maintaining medical information and filing/scanning, maintain sample supplies, ordering and maintaining clinical supplies as needed, patient education, obtaining histories and vitals from patients, triage patient calls, prescription refills, call backs for normal or abnormal results as per physician direction, prior authorizations as needed, telephone answering and appointment scheduling as needed. JOB DUTIES Prepares patients for provider visits according to approved policies and procedures i.e. reason for visit, history (past medical, family and social), symptoms, problems, weight, height, vital signs, medications, allergies. Administers medications to patients as regulated by the Nurse Practice Acts and as ordered by the provider. Reviews and updates medication lists identifying any possible ineffective drug therapies, adverse reactions, side effects, drug allergies and contraindicated medications. Assists Registered Nurses Physicians with procedures as requested. Appropriately prepares patients for procedures or debridement, records procedure, patient response, wound documentation with measurements. Performs ABI’s as ordered, multilayer wraps, application of NPWT. Notifies patients of lab and test results under direction of Registered Nurse. Collects specimens as directed and performs limited lab tests on site as ordered by the provider. Maintains equipment and controls including refrigerator temperatures if applicable. Responsible for patient and equipment safety. Communicates and interacts effectively with patients and families, physicians, peers and other health team members to maximize successful patient outcomes. Patient education. Assists in the facilitation of smooth patient flow. Greets patients and families courteously. Maintains good communication with providers and coworkers. Assures adequate amounts of medical supplies and medications are available and stocked appropriately. Maintains inventory of inventory and rotates stock to make sure oldest is used first. Also, checks expiration dates monthly. Assures nursing documentation in the medical records is complete wound photographs and measurements. Performs administrative duties as requested. To include but not limited to: Appointment scheduling and follow up appointment scheduling as needed, telephone etiquette including message taking and recalling patients, picking up, opening and distributing mail, obtain authorizations as needed. Maintains exam rooms and workstation and ensures cleanliness. Maintains the organized flow of nursing information and documentation. Documents are organized and available for use when needed. Schedule tests or procedures for patients as appropriate. Maintains medical information and filing and or scanning. Use of Meditech, Wound Expert EMR. WORKING CONDITIONS Work setting will be in an outpatient wound clinic involving the ability to bend, stoop, lift, twist and maneuver as necessary to conduct patient evaluations. Possible exposure to various chemicals relating to job performance will be in use of equipment and in all clinical/medical procedures which may be performed. Use of computers, printers, fax machines, copiers, telephone and paper shredders. Must have ability to exhibit independent judgment skills and show initiative. Ability to use tact and poise in representing MMC. Work with minimal direction and assistance and frequent interruptions. Must be able to multitask. Must be able to work first shift but there maybe times that there will be additional hours that will be needed on short notice. Ability to manage multiple priorities under stress. Flexibility to adapt to changing situations in a positive, professional manner. JOB SUMMARY Provides all necessary services needed to assist hospice patients to live in the Marquette Hospice House, becoming part of the Hospice team to carry out the patient’s plan of care. JOB DUTIES Provides direct care to guests (hospice patients) who live in the home. Assist in the facilitation of smooth patient transition, greets guests and their families courteously. Prioritizes patient care. Provides all identified needs of our guests under the direct supervision of the Hospice RN who operates under physician orders. Administers medication to guests as regulated by the nurse practice acts and in accordance with guests’ plan of care. Manages medication box and medication audits. Maintains the organized flow of nursing information and documentation. Answers all phone calls courteously and relays messages appropriately. Provides assistance with all ADL’s. Supports the hospice philosophy. Provides family education while working with the interdisciplinary team. Provides emotional support for our guests and their visitors. Responds to guest and visitor requests and complaints with immediate notification to direct supervisor or designee. Completes daily household chores to ensure home is clean i.e. laundry, dusting, sweeping, dishes, etc. Works cooperatively with the volunteers that will be helping with household chores. Maintains a safe environment for our guests and their visitors. Demonstrates knowledge of emergency procedures. Plans and prepares meals, snacks, and supplements as needed. Maintains close communication with RN supervisor regarding guest status. Notifies RN of any changes in guests’ condition in a timely manner. Performs other duties as assigned.
