Reports potentially unsafe equipment to the nurse on duty and to the DON immediately. H. Assists with the distribution of clinical alerts to other staff that are working with certain focused residents/patients. I. Answers call lights promptly. J. Assists in the updating of the fall prevention intervention log (which includes visual interventions. K. Meets with new admissions and reviews primary safety measures. L. Reports any changes in resident/patient condition to the nurse on duty. M. Secures incident/accident reporting from the nurses upon arrival for duty. N. Checks to ensure that new interventions are promptly implemented post occurrence. O. Communicated trends/patterns, and individual resident/patient safety concerns to the IDT through Stand-Up Meeting (day and evening shifts), and during early mornings for night shift. P. Communicates with the CNAs to assists with determining the root cause or new behaviors associated with incidents/accidents, and potential new interventions based on the root cause. Communicates this information to the restorative nurse. Q. Meets with DON and corporate to review observations and recommendations for the incident/accident prevention program. R. Attends and actively participates in resident/patient care plan conferences as directed by the IDT team. S. Attends and participates in facility Incident/Accident related meetings. T. Attends educational sessions related to resident/patient safety and shares educational information with the IDT. U.
