Residential Program Nurse (RN) - Brain Injury Services

Overview Starting Rate: $46.50/hour The Residential Programs Nurse provides direct nursing oversight and management of medical and care components in accordance with Advocates and funder requirements. The Residential Program Nurse provides training, supervision, and oversight for both residents and staff in all aspects of medical and medication management. They will collaborate with the Nursing Director, Residential Program Directors and nursing team in the implementation and follow-through of the individualized service plans and documentation in accordance with the mission and regulations of the program. Minimum Education Required Bachelor's Degree Responsibilities Responsibilities include, but are not limited to: Take a lead role in assessing potential program participants at his or her residential site; identify the program participant’s needs and functional abilities. Coordinate assessment for new durable medical equipment needs. Provide oversite and ensure on- going equipment checks and regularly scheduled maintenance. Participate initially and at least annually in a team assessment with the applicant, family member / guardian, and Residential Program Director as to whether the applicant can manage their personal care services. Complete the annual assessments related to the program participant’s support needs. Ensure Physician’s Authorization for each GAFC resident twice annually per Medicaid site policies (if applicable). Prior to an applicant’s admission to the program, complete a Medication Assessment and develop an Emergency sheet, Medication List, nursing care plan, guidelines, protocols, individualized ADL sheet and coordinate with the Residential Program Director to complete a risk plan. Develop an individualized Emergency Contact sheet, which includes family, guardian, physicians’ (PCP and specialists), clinic/hospital contact information, Social Security Number, MassHealth ID number, date of birth, gender, guardianship, health care proxy, marital status and code status, and program start date for each new resident prior to his/her start at the program. Assist residents with all aspects of medical management including physicals, referrals, prescriptions, appointments, and treatments. Coordinate with residents, family members, physicians, other medical providers, and site team regarding all medical care and changes in status, treatment, or medication. Implement, oversee, and supervise medication management and best practices in accordance with agency policy relating to MAP in a manner consistent with funder regulations. Provide supervision and oversight in Supported Living Program (SLP) programs and nursing team in coordination with the Nursing Director. Provide and document person-centered training to direct care staff as part of the nursing team. Develop and implement trainings as part of the nursing team as needed. Promote and maintain a safe environment: Complete all documentation in a timely manner to ensure the availability of accurate information in accordance with established standards. Monitor and evaluate resident care and generate ideas for quality improvement. Remain available on call 24 hours per day, 7 days per week. Schedule and notify off days with Nursing Director to ensure on call coverage. This position is 40hrs, weekend and holiday requirement, opportunities available at MGH main campus The MGB Cancer Institute is seeking a full-time Nurse Practitioner (NP) or Physician Assistant (PA) to join the Infusion Unit and ambulatory practices at Mass General Hospital in Boston, MA. The Infusion Unit APP Specialist will work in a recently developed role embedded within our adult infusion unit to serve as an infusion unit first responder, promote infusion services access and efficiency, provide oncology urgent care/symptom management and provide generalist coverage within our ambulatory solid tumor disease centers or hematologic malignancies programs for anticipated leaves and absences. Join a large and supportive ambulatory oncology and infusion service line Advanced Practice Team in a role that specializes in the care of patients with a variety of medical oncology diagnoses. With an interdisciplinary team approach, this provider will promote a collaborative relationship between providers, professional nurses, research personnel and administrative staff, to provide safe, quality care in compliance with clinical trial protocols across the care continuum. APP Responsibilities: • While on the adult infusion unit, the APP oversees the care of patients with hematologic and oncologic disorders, serves as a first responder for acute reactions and medical emergencies, partners with primary disease center providers to care for complex oncology patients awaiting hospital admission and collaborates with the Cancer Institute weekend first responder team to provide access for urgent evaluation and symptom management during off-hours. • The NP/PA is also responsible for collaborative care of the patient with benign hematological disorders receiving care in the adult infusion unit. • While in the cancer institute ambulatory practices the APP Specialist is responsible for the care of oncology patients in the outpatient setting, specializing in either solid tumor malignancies or hematologic malignancies in partnership with primary disease center APPs, physicians, practices nurses and administrative support team members. • Flexibility in scheduling as this position may require weekend, holiday and evening coverage as well as variable weekday assignments. Education/Certifications: • Master’s Degree in Nursing or Master’s Degree from a NCCPA accredited PA program required • Current licensure in the commonwealth of Massachusetts as a certified NP/PA required • If NP, certification may be Adult, Acute or Family Nurse Practitioner • If PA, national certification by the NCCPA • DEA and MCSR is required • ACLS certification is preferred Experience: • 3 years’ APP experience in oncology is preferred.
