Job Responsibilities

Providing nursing care in post-acute care facilities
Checking vital signs such as blood pressure and pulse and providing regular reports to the broader nursing team
Observing the conditions of residents on an ongoing basis, alerting supervising staff to any concerns or emergencies

Adheres to the Nurse Practice Act of NH. Assist with admissions process within the Electronic Health Record (EHR), collaborating with the House Supervisor (HS) and Charge RN. Inform the primary RN when the admission orders are entered for hand off report. Assemble and label patient chart. Place prior advanced directive on chart.
