Location: Fort Wayne Medical Oncology and Hematology Pay Range: $24.83 - $46.12 Position Summary Provides safe, accurate and efficient patient care, including but not limited to: patient assessment, review of labs and vital signs, medication safety, dose verification, and drug administration. Follows all Job Breakdown Instructions (JBIs) associated with role. Provides triage care when necessary. Communicates patient condition effectively to multidisciplinary team members. Key Performance Areas Safely and accurately administer chemotherapy, immunotherapy, biotherapy, supportive care, and non-oncologic agents to patients as ordered by providers. Will use Oncology Nurse’s Society (ONS) standards and guidelines. Accurate and safe disposal of biohazardous waste, per OSHA and NIOSH guidelines. Responsible for new and ongoing patient education, continual patient assessment, and support. Documentation: Thorough, accurate and timely documentation in the Electronic Medical Record (EMR) of treatment rendered to patients. Ensure appropriate charges are captured by following guidelines for appropriate administration of all medications and activities. Will follow proper patient identification per JBI for all patient encounters. Teaching: Educate patients and caregivers about medications, side effects and planned treatment course. Interact with patient and family in a caring, professional manner. Telephone triage: Includes prioritizing incoming calls and messages from patients, family members, hospitals, nursing facilities, pharmacies, and physician offices from emergent to non-urgent. Must respond accordingly in an appropriate time frame. Supplies: Includes assisting with inventory and re-stocking medical supplies, recommend purchase of new items as needed. Place stock in storage areas in a timely manner. Complete annual required safety and nursing trainings. Attend all mandatory staff meetings as necessary. Always maintain and ensure the confidentiality of all patient and employee information. Assist in training new employees related to job duties according to JBIs. Will be expected to work at any Company location to help meet the Company’s business needs on an as-needed basis, within reasonable expectations. Will also be expected to work overtime when given sufficient notice. Performs other duties and projects as assigned. Complies with all federal and state laws and regulations pertaining to patient privacy, patient’s rights, personnel law, safety, labor, and employment law. Certifications/Licenses Unencumbered RN License in state of employment or multi-state license. Current CPR and BLS certifications. Valid Driver’s License and vehicle for travel to other clinic locations as needed. Previous Experience Experience with hematology/oncology patients in a clinic or hospital setting. Ability to assess patient needs and condition is essential. Must have strong critical thinking skills, as well as the ability to react calmly and effectively in emergency situations. Fluency in the English language with excellent oral and written communication skills is required. Summary The Student Nurse Apprentice - RN is responsible for assisting the designated nurse preceptor and additional nurses with care of all age groups within the Standard of Care set by Parkview Health and the Indiana Nurse Practice Act. Position Overview: Coordinate a care service line for a high-acuity patient population within the department Conduct extensive assessments and exercise clinical judgment on physiological health care Monitor patient status and develop appropriate care plans in collaboration with the healthcare team Serve as a clinical expert and strong unit leader, supporting unit and system leadership activities Apply advanced clinical knowledge of the specialty; required to scrub and/or circulate Develop a core curriculum for the service line in partnership with the clinical educator and system education team Oversee orientation for new staff and ongoing staff education and program evaluation within the service line Act as a role model, resource person, and member of the management team Ensure all activities align with the organization’s Mission and Vision Qualifications: Requires an Associates of Nursing (ASN). Bachelors of Nursing (BSN) preferred. Requires an active Registered Nurse (RN) license in the state of Indiana or an active Nurse Licensure Compact (NLC) RN license. Requires that the RN has graduated from a nationally accredited nursing program. Requires Basic Life Support certification through the AHA. Other advanced life support certifications may be required per unit/department specialty according to patient care policies. 
The RN/LPN Care Navigator coordinates the admitting and transferring of residents for Adams Woodcrest, Adams Heritage, AND Adams Memorial Swing Bed facilities. Strives to provide an environment conducive to resident safety and comfort. Demonstrates competence in general nursing skills and is conversant in Medicare, Medicaid, and State/Federal guidelines for long-term care facilities. Maintains a working relationship acting as liaison with discharge planners, insurance carriers, physicians, and families. Maintains compliance with initial prior authorization for resident care. Must have a willingness to have flexible hours to meet the needs of the hospital and nursing facility. Develops a department budget in coordination with the executive director . Performs other duties as assigned by Director of Senior Living . This position is eligible for a sign-on bonus - RN: $20,000 / LPN: $12,000 Eligible for medical coverage on your 1st day of employment, all other benefits will be effective the 1st of the month following hire date! Requirements: 40 hours per week - First Shift 8:00 AM - 4:30 PM On - Call Every Other Weekend Valid RN or LPN license in the State of Indiana BLS/CPR Certification Required A Registered Nurse (RN) or Licensed Practical Nurse (LPN) in a physician's office plays a vital role in patient communication and care coordination. They handle incoming calls, direct messages to the appropriate healthcare providers, send prescriptions to pharmacies, and follow instructions related to referrals, lab tests, or treatment orders. Additionally, they assist in preparing patients for examinations, help with scheduling appointments, and provide flexible support across various tasks as needed, ensuring smooth clinic operations and quality patient care. Eligible for medical coverage on your first day of employment, all other benefits will be effective the 1st of the month following hire date! Assesses hospice care patients and families to identify the physical, psychosocial, and environmental needs of patients as evidenced by documentation, clinical records, Interdisciplinary Team reports, after hours reports, and on-site evaluations. Assesses patient needs and obtains data on physical, psychological, social, and spiritual factors that may influence patient/family/caregiver health status. Communicates significant findings, problems, and changes in health condition, environment, or unsafe facility conditions to the Clinical Supervisor, physician, facility, and/or other personnel involved with patient care. Counsels, instructs, and includes the patient, facility, and family in following the Interdisciplinary Plan of Care. Teaches hospice philosophy and pain and symptom management to staff in alternate care settings (nursing homes, hospital, assisted-living facilities, etc.). Provides appropriate support at time of death and perform bereavement assessment. What is Required? Graduate of an accredited Certificate, Diploma, Associate, or Baccalaureate School of Nursing Current State License as a Registered Nurse RN 1 year of experience in a clinical care setting Experience in a hospice or home health environment is preferred 50% travel required Sit, stand, bend, lift and move intermittently and be able to lift 50-100 lbs.
