Job Summary The Ambulatory Care RN is accountable for the delivery of direct patient care including patient education and the management of telephone triage in a clinical setting. The RN supports professional nursing practice across practice settings and across the continuum of care to meet the needs of the patient and family; assesses the needs of the patient undergoing surgical/diagnostic/therapeutic procedures; plans, implements, and evaluates patient care; and teaches patients, families, orients colleagues and peers. Replaces job #274419 Responsibilities* Accountable for the delivery of direct patient care including patient education, symptom management, and telephone triage in a clinical ambulatory care setting to adult bone marrow transplant and leukemia patients. Supports professional nursing practice across practice settings and across the continuum of care to meet the needs of the patient and family; assesses the needs of the patient undergoing surgical/diagnostic/therapeutic procedures; plans, implements, and evaluates patient care; and teaches patients, families, orients colleagues and peers. Candidate will provide coverage within two nursing work teams in the BMT and Leukemia clinics. The candidate must have demonstrated the following in his/her current position: Competent Performance Accountable for the management of patient care assigned in an ambulatory care setting. Utilizes critical thinking skills to recognize and solve patient problems. Utilizes the nursing process and nursing standards to prevent or manage actual/potential patient problems in the clinical oncology setting. Identifies patient/family learning needs and ensures that they are met. Serves as a patient advocate. Works with the various Cancer Center associates to ensure patients are scheduled appropriately. Communication And Respect Maintains a highly professional and positive attitude regardless of workload and stress level. Inputs, retrieves and interprets data from multiple manuals and computer-based information systems. Responds to voice mail and electronic communication with the patients and colleagues in a timely manner. Exhibits professional verbal and non-verbal communication skills. Professional Leadership Completes assigned duties in an efficient manner and utilizes time effectively. Performs procedures in a safe, efficient and cost-effective manner. Participates in meeting the educational needs of self and others. Completes all other clinical and administrative duties as assigned. Implements change to achieve team goals. Seeks opportunities to meet or exceed individual objectives identified in the Professional Framework Model. Team Work Uses organizational skills to ensure the team provides safe, efficient care to the patient and family. Uses organizational skills to assure continuity of care in collaboration with other disciplines through appropriate planning, education and coordination of services. Delegates to other health care team members according to the Nurse Practice Act. Collaborates with medical staff to facilitate and optimize the care of the patient. Research And Quality Improvement Acts on behalf of patients and families to ensure care is safe and appropriate with consideration for values, diversity and human rights. Participates in nursing and medical research and facilitates transfer of new knowledge to patient care. Participates in the Quality Improvement Process. Requirements of the Commission on Cancer Certification must be met within the triannual certification window proportionate to hire or transfer to the oncology area, details provided by leadership. Required Qualifications* Current licensure as a Registered Nurse to practice in the State of Michigan. Two (2) years of recent experience managing care for the Hematology/Oncology patient population. NOTE: Required qualifications must be met by the candidate in order to be interviewed and considered for the position. Candidates who are not qualified will not be interviewed. Actual Nursing Framework LEVEL and salary will be determined at time of hire. Nursing Framework levels range from Level A to Level F.
