Job Responsibilities: Manage all aspects of patient care from initial admission assessment to weekly visits, to discharge when goals are met. Coordinate patient care with their physician as well as other disciplines involved in care (PT,OT,ST,HHA) Upon admission, develop plan of care tailored to patient’s individual needs, including establishing goals and discharge planning. Revise the plan of care as necessary, depending on patient status and any changes. Communicate with physician regularly on patient condition and to request any orders necessary. Provide direct nursing care and education based on the care plan. Completes all assessments and nursing notes timely and thoroughly. Collaborate with other home health disciplines for the best patient outcomes, which includes attending case conference meetings regarding patient overall care. Supervision of HHA and LPN visits, when applicable. Communicate effectively with other team members, including administrative supervisors regarding scheduling, patient updates, or any other daily updates. Participate in on-call rotation when applicable. Qualifications: Graduate of an accredited school of nursing with Associate/Bachelor’s degree. Current unencumbered LPN license and CPR certification 1 year of home health experience preferred, but willing to train the right candidate. 1-2 years of acute care LPN experience required.
