Records observations, treatments, and other pertinent information. Communicate with Medical Director, Attending Physician, branch leadership and IDG regarding findings from the visits and any measures to alleviate symptoms. Provides physical and emotional care to support the patient and family in period of crisis. Ability to triage consults by patient acuity. Performs bedside palliative care with patient/family education Ensures that all activities performed align with the vision of Elara Caring’s board of directors, executive team, and the leadership of the Hospice team. Prevents unwanted hospitalizations by meeting patient and family needs in the home. Prepares and maintains accurate patient records, charts, and documents to support sound medical practice and reimbursement for services provided. Compliance with applicable laws and regulations with respect to Collaborative Agreements. Completes accurate billing and coding activities for all patient encounters according to CMS guidelines and initiates reimbursement for medically necessary visits in accordance with applicable law and regulation. Provides training and continuing education for staff. Assists in development of clinical practice guidelines/standards in support of quality palliative care. Adheres to the practice of confidentiality regarding patients, families, staff and the organization. Collaborates/Communicates with Medical Director, attending physician, branch leadership and IDG. Performs other duties/projects as assigned. What is Required? Master’s degree in nursing and certification through a national Nurse Practitioner Licensing board. 1+ years of clinical nursing experience 1+ year post-graduate clinical nursing experience in the hospice and palliative care field preferred Prior experience in Family Medicine, Geriatrics and psychiatry preferred. Current licensure in the state of practice as a Nurse Practitioner and maintain such licensure in accordance with applicable laws and regulations. Maintain active DEA certification if state allows prescription of controlled substances by APPs 50% travel required Sit, stand, bend, lift and move intermittently and be able to lift 50-100 lbs. We are hiring a Nurse Practitioner to join our care team with Hospice of Western & Central Massachusetts! This is to assist with on-call admissions visits. Candidate must be willing to travel. $100 a visit. Must be available within 48 hours to perform face-to-face visits. The nurse practitioner provides comprehensive hospice services and care through the evaluation and assessment of the health history, health promotion, diagnosis of medical conditions, management of health problems, prescribing medications or treatments, and other duties. Experience & Education: Master's degree with a minimum of one (1) year of experience as a nurse practitioner; hospice or palliative care experience preferred. CPR certified. DEA licensed. Accreditation from an approved certifying body for advanced practice nursing. Preferred certification in a specialist area such as hospice or pediatrics. Duties & Responsibilities: Provides medical care, palliates, and manages terminal illness in compliance with the state Nurse Practice Act under the supervision of a collaborating physician. Completes history and physical examinations by evaluating the patient's medical condition and health history, ordering, and interpreting results from lab and diagnostic tests or procedures, diagnosing health conditions and documenting the findings in the patient's medical records. Manages health conditions and terminal illnesses by developing plans of care, prescribing medications and/or treatments, obtaining consultation or making referrals and coordinating hospice care services. Makes recommendations to the hospice medical director regarding patient eligibility for certification and recertification for hospice care. Reviews clinical information, and coordinates and oversees patient care in consultation with the hospice interdisciplinary team and other healthcare practitioners. Completes, maintains, and submits accurate and relevant clinical notes, physical examination notes, assessment visit notes, medical orders, collaboration notes and other documentation in the medical record. Ensures compliance with the Medicare conditions of participation and other state regulations governing the provision of healthcare. Builds from the resources in the community to plan and direct services to meet the needs of individuals and families.
