Consults with the primary nurse and other disciplines involved in the patient’s care. 5. Prepares clinical and progress notes and documents appropriately according to company policy for all assigned patients. Provides thorough observation of patient status and reports change to Hospice Director or On Call Nurse for appropriate action. 6. Meets the requirements for participation in patient/community/referral source education plan as stipulated by agency policy. 7. May administer sub q and IM medications according to company policy. 8. Attends all in services prior to performing new policies/procedures. 9. Consults RN/Clinical Coordinator regarding any unfamiliar policies/procedures. 10.
