Responsibilities:

• Provide high-quality home care services to clients, including assistance with daily living activities, personal care, and homemaking tasks
• Develop and implement personalized care plans in collaboration with clients, families, and healthcare providers
• Monitor and report client progress, identifying areas for improvement and implementing changes as needed
• Maintain accurate and detailed records of client care, including medical histories, treatment plans, and medication administration
• Work closely with clients' healthcare teams to ensure seamless transitions and communication
• Demonstrated ability to work independently, with minimal supervision, while maintaining a strong focus on client care and safety

Requirements:

• 2+ years of experience in home health care, hospice care, or a related field
• High school diploma or equivalent required; Associates or Bachelor's degree in a related field preferred
• Certification in Basic Life Support (BLS) or CPR; willingness to obtain certification within 6 months of hire
• Ability to work flexible schedules, including weekends and holidays; willingness to engage in on-call duties as needed
• Reliable transportation and a valid driver's license (if applicable)
• Ability to lift, bend, and move objects weighing up to 50 pounds; ability to remain on feet for extended periods
• Excellent communication, organizational, and interpersonal skills
